POSITIONSUMMARY: TheAssisi Bridge House (ABH) is based on 12 recovery domains in cooperation withthe program of Alcoholics Anonymous/ Narcotics Anonymous. The Case Manager is responsible for workingwith the treatment team to connect clients to the services needed and be a linkto resources in the community outside of ABH.
ESSENTIALDUTIES AND RESPONSIBILITIES INCLUDE THE FOLLOWING:

Bea liaison with Medicaid and SCLHSA providers to preauthorize and authorizestays for the clients at ABH based on their individual needs. This requires anunderstanding of diagnoses codes ASAM criteria client progress familyinvolvement and discharge planning.
Beable to manage the waiting list in regards to priority placement andexclusionary criteria. This also includes referring callers who do not meet thecriteria of ABH to appropriate care.
Makereferrals for clients to resources including but not limited to behavioralhealth medical appointments dental and eye care appointments job placementcenter LRS parenting classes smoking cessation and housing among others.
Makefollowup calls to former ABH clients to determine if they have one year cleanand sober and obtain information regarding their status at that time (jobhousing etc.
Havea good understanding of charting.
Assistwith research and calls for building repairs maintenance and renovation.
Participatein group and individual sessions as needed/required to support clients in theirrecovery efforts.
OTHEROR SHARED RESPONSIBILITIES:
Assistthe administrator and clinical director with other program or administrativeduties as requested.

QUALIFICATIONSTO PERFORM THE JOB SUCCESSFULLY:
Educationand/or Experience:
Collegedegree preferred.
Peersupport specialist certification preferred.
Must befamiliar with all dynamics of alcoholism and drug abuse and the variousprograms for treating the disease.
Driverslicense required.
OtherSkills required:
Allemployees of ABH and ILP must maintain strict confidentiality al all times.
