Under direction The Substance Use Navigator (SUN) is a key team member responsible for connecting patients with evidencebased treatment for Substance Use Disorders (SUD). With a focus on reducing barriers to Medication for Addiction Treatment (MAT) and promoting harm reduction strategies the SUN works closely with hospital staff patients and community providers. This role emphasizes patient engagement advocacy and care coordination to foster positive health outcomes. Exercises no supervision.

Examples of Duties

Examples of duties may include but are not limited to the following:

  • Engage individuals using drugs in nonjudgmental supportive and informative conversations to encourage participation in harm reduction strategies and SUD treatment opportunities;
  • Conduct brief initial assessments to identify patient needs and readiness for treatment;
  • Communicate daily with hospital staff to recruit patients and provide behavioral health and MAT support;
  • Identify existing primary care and behavioral health providers for patients and coordinate care;
  • Connect patients with available MAT programs at current primary care providers or behavioral health locations when appropriate;
  • Facilitate scheduling of appointments and transitions to outpatient MAT clinics and community resources;
  • Provide comprehensive patient education and counseling on MAT options including medications such as methadone buprenorphine naltrexone bupropion mirtazapine and gabapentin;
  • Serve as a patient advocate promoting stigmafree compassionate care in hospital and community setting;.
  • Assist patients in accessing harm reduction tools and services including mental health care financial counseling social services and residential treatment facilities;
  • Build trust with patients to encourage longterm engagement with care;
  • Collaborate with hospital staff and community organizations to promote culture change around the treatment of individuals with SUD;
  • Participate in team training to stay updated on best practices for harm reduction and MAT;
  • Performs related duties and responsibilities as assigned.

Knowledge Skills and Abilities
Knowledge of:

  • Principles and procedures of record keeping and reporting;
  • Principles and practices used in dealing with the public;
  • Safe driving principles and practices;

  • Exercise good judgment and make sound decisions with no or little supervision;
  • Understand and follow oral and written instructions;

  • Analyze situations carefully and adopt an affective courses of action;
  • Interact effectively and sensitively with individuals from diverse backgrounds;
  • Display compassion understanding and empathy towards those in need of services;
  • Speak clearly and distinctly in a wellmodulated voice;
  • Maintain confidentiality of sensitive information;

  • Hear and recognize conditions or circumstances that indicate something might be wrong unusual or out of the ordinary;
  • Communicate clearly and concisely both orally and in writing;
  • Interact with individuals who are mentally ill and developmentally disabled; and
  • Establish maintain and foster positive and harmonious working relationships with those contacted in the course of work.

Qualifications

Experience:

  • At least one year of direct patient care experience in substance use disorders and harm reduction within the last two years OR current fulltime employment of at least three months as a Substance Use Navigator including experience distributing Medication for Addiction Treatment (MAT) and overdose recognition and response.
Education:
  • High school diploma or equivalent.
  • Alcohol and drug counselor license (CAADAC) is desirable.
Licenses and Certificates:
  • Possession of or the ability to obtain a valid California drivers license at the time of appointment.
Special requirements:
Essential functions and duties require the following physical abilities and work environment:
  • Ability to travel to different sites and locations;
  • Regularly required to talk and hear to sit stand walk run kneel crouch stoop squat crawl twist climb bend and lift up to 50 lbs;
  • Exposure to heat cold noise outdoors vibration odors and chemicals;
  • Ability to work flexible hours weekends and holidays.
