Our longterm care facilities at PHC
Providence Health Care operates six Longterm Care Homes and one Assisted Living Care Home for seniors providing 24hour nursing care and a broad range of medical and personal assistance to support social / leisure needs and activities of daily living. The homes are located throughout the lower mainland with staff working 8hour shifts.
Are you a newly graduated RN or an experienced professional Providence Health Care is seeking dedicated Registered Nurses to join our 6longterm care facilities.

- St. Vincents: Langara at 255 W62nd Ave Vancouver BC
- Youville Residence at 4950 Heather St Vancouver BC
- Holy Family Hospital at 7801 Argyle Dr Vancouver BC
- Mount Saint Joseph Hospital at 3080 Prince Edward St Vancouver BC
- Chenchenstway at 7550 Rosewood St Burnaby BC
As a Staff Nurse youll be at the forefront of patient care working collaboratively within an interdisciplinary team to ensure patients/residents receive comprehensive support. Your responsibilities will include:
- Assess patients physical social spiritual and psychological needs
- Develop implement and evaluate individualized care plans
- Incorporate teaching into care plans to educate patients and families
- Make referrals to resources as needed
- Support fellow healthcare professionals
- Operate within the framework of Providence Health Cares mission vision and values

What You Bring
Education:
- Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives
Skills and Abilities:
- Demonstrated competence in performing CPR procedures.
- Experience in teamwork decisionmaking and conflict resolution
- Demonstrated a teaching aptitude
- Proficient in clinical leadership
- A strong desire to expand your professional skills and a commitment to lifelong learning in the nursing field

Your Day to Day:
- Conduct thorough resident assessments through chart reviews observations interviews and collaboration with peers
- Develop implement and evaluate nursing care plans based on evidencebased practices prioritizing resident needs
- Adhere to established policies procedures and PHC standards while providing highquality nursing care
- Monitor resident progress evaluate outcomes and adjust care plans as needed for optimal results
- Maintain accurate records of assessments care plans patient/resident status and discharge plans using manual and computerized systems
- Identify and address resident and family learning needs providing education and referrals as necessary
- Provide direction to staff promote teamwork and share clinical knowledge to enhance patient care
- Support nursing research initiatives and contribute to the improvement of resident care policies and procedures
- Advocate for residents rights and dignity facilitate discharge plans and collaborate with families and community resources
- Perform additional duties as assigned to support the healthcare team
