Job Summary:
This Registered Nurse (RN) position requires providing skilled nursing care case management and hospice care to patients with advancing age frailty and serious illness. The RN will perform patient assessments administer medications develop treatment plans and collaborate with the interdisciplinary team. Strong communication and collaboration skills are essential.

Responsibilities:
- Patient assessments and monitoring.
- Medication administration and documentation.

- Collaboration with physicians other nurses and healthcare professionals.

- Maintaining accurate patient records.

Required Skills & Certifications:
- Active Registered Nurse (RN) license in the state of employment.
- Bachelors degree in Nursing or related field.
- Minimum two years of experience in hospital home health or hospice care.
- Clinical assessment medication administration and treatment planning skills.
- Interdisciplinary team collaboration skills.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- Drivers license car insurance and personal vehicle for field work.
Preferred Skills & Certifications:
- Experience in Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU or Operating Room settings.
