Job Description

ESSENTIAL DUTIES 

The essential functions of a Health Worker IV (2588) include but are not limited to: 

  • Supervise and train lowerclassification paraprofessional personnel in the skills required for successful performance of their assigned duties
  • Assist in the preparation of inservice training materials
  • Communicate with Team Leaders regarding the function and performance of Health Workers I II and III
  • Meet with representatives of the local community professional organizations and social agencies to inform them of available treatment programs and to elicit their cooperation and support
  • Provide community education services
  • Assist in the planning and execution of programs and operations research projects and reports as directed
  • Evaluate data and suggest programs in the preventive areas of social and health adjustments
  • When assigned to a specialized activity program supervise the program on specified wards or units and/or assist the program director
  • Supervise specific health or rehabilitation programs as needed
  • Plan direct and coordinate the educational recreational therapeutic and work projects of program participants as directed by professional staff
  • Interview and screen program clients identifying general client conditions
  • Perform crisis intervention activities under professional direction
  • Provide broad social counseling services and assist in group therapy sessions
  • Perform other duties as assigned

Qualifications :

MINIMUM QUALIFICATIONS 

1. EXPERIENCE: Three (3) years of verifiable experience within the last five (5) years performing a combination of at least two (2) of the following duties:

  • Serving as a liaison between targeted communities and healthcare agencies
  • Providing culturally appropriate health education/information and outreach to targeted populations
  • Providing referral and follow up services or otherwise coordinating care
  • Providing informal counseling social support and advocacy to targeted populations
  • Escorting and transporting clients;
  • Providing courier/dispatcher functions
  • Performing preclinical examinations of vital statistics such as measuring a patients weight height temperature and blood pressure

Substitution: Possession of a Community Health Worker Certificate from City College of San Francisco can substitute for 6 months of the required experience. 

    SPECIAL CONDITIONS: Depending on the departments needs some positions may require applicants to meet additional special conditions which may include but are not limited: 

    • Bilingual Proficiency in a variety of languages. Only those eligible candidates who pass the bilingual proficiency test will be considered for bilingual positions
    • Experience with HIV AIDS and Sexually Transmitted Diseases (STDs)
    • Experience providing health or social services to diverse populations

    One year of fulltime employment is equivalent to 2000 hours. (2000 hours of qualifying work experience is based on a 40hour work week.)

      Assessment Component

      Candidates that meet the minimum qualifications will be invited to participate in a Multiple Choice exam that is designed to measure the knowledge skills and abilities in job related areas including but not limited to: 

      • Knowledge of the ethnic economic and social factors affecting residents of the neighborhood served by the health program
      • Knowledge of the physical and emotional symptoms and resources available for their resolution
      • Knowledge of community services agencies and health program principles
      • Knowledge of group methods and techniques of mental health education
      • Ability to speak read and write English as well as the predominant language of the district served
      • Ability to assume a high degree of responsibility and undertake supervisory duties
      • Ability to provide effective liaison between staff patients neighborhood residents and programs involving these groups

      Written Exam:(Weight: 100%) 

      Candidates must achieve a passing score on the written multiplechoice exam in order to continue in the selection process and will be placed on the confidential eligible list in rank order according to their final score. 

      Additional selection processes may be conducted by the hiring department prior to making final hiring decisions.

      Certification
      The certification rule for the eligible list resulting from this examination will be the Rule of 10Scores.

      Eligible List/Score Report:
      Once you pass the exam you will be placed onto an eligible list and given a score and a rank. For more information visit  duration of the eligible list resulting from this examination process will be 12 months and may be extended with the approval of the Human Resources Director.
