Overview
We are seeking a passionate and dedicated Clinical Pharmacist to join our healthcare team. As a Clinical Pharmacist you will play a crucial role in ensuring the safe and effective use of medications for our patients. You will work closely with other healthcare professionals to provide comprehensive medication management and optimize patient outcomes.
Responsibilities
- Collaborate with healthcare providers to develop and implement medication therapy management plans
- Conduct medication reviews and provide recommendations to optimize drug therapy
- Educate patients and caregivers on the safe and appropriate use of medications
- Participate in multidisciplinary rounds and provide clinical input on medicationrelated issues
- Utilize pharmacy and medical software systems such as Emis and Docman to document and track medicationrelated tasks
- Perform medication reconciliation and ensure accurate and complete medication histories
Qualifications
- Degree in Pharmacy from an accredited institution
- Registered Pharmacist with relevant licensure
- Experience in medication management and review
- Strong knowledge of pharmaceutical and medical software systems including Emis and Docman
- Excellent communication and interpersonal skills
- Ability to work effectively in a multidisciplinary healthcare team
Daytoday
- Collaborate with physicians and nurses to review and adjust medication therapy plans
- Educate patients on their medication regimens and answer their questions
- Use Emis and Docman to document medicationrelated tasks and track patient medication history
- Participate in meetings and rounds with other healthcare professionals to discuss patient care and medication management
- Stay updated on the latest developments in pharmacotherapy and medication management practices
