Kyabram District Health Service (KDHS) is a mediumsized rural health service that delivers a range of programs and services including acute subacute residential aged care and allied and primary health care services. KDHS has three campuses located in the townships of Kyabram Stanhope and Tongala.

Our Community Care Workers provide a range of high quality services aimed providing support and assistance to people living in their own homes in their daytoday living ensuring that they have maximum opportunity for developing valued and valuing lifestyles. They will provide opportunities for choice selfadvocacy and participation in matters relating to the home as well as facilitating access to community services and facilities. To support client with all aspects of everyday living personal care washing cleaning laundry preparing and cooking meals and shopping.
