Job Description

General Summary:

The Manager Care Coordination Clinical Operations (CM) is a highly skilled care coordinator focusing on the improvement of quality and safety outcomes of patients in the care coordination program. The CM is a healthcare system specialist that oversees patient care coordination assesses health risks participates in the development of quality improvement strategies facilitates team communication educates and orients team members engages patients and families and implements evidencebased solutions. The CM helps plan and coordinate complex patient care developing nurses social workers and community health workers through novice to expert levels of practice.

Demonstrates clinical expertise in coordinating care for children with medical behavioral health and social complexities. Performs as a clinical expert and educator within the scope of practice in the assigned area. Functions as a professional care coordinator and applies an extensive background of knowledge and experience with infants children adolescent and adult patients displaying mature critical thinking skills. Acts as a leader in improving and advancing care coordination practice through analytical and creative measures; accountable for high standards of professional practice and fosters accountability in others. Acts as a leader who has earned the trust of peers and managers and has achieved recognition for care coordination excellence.

Essential Job Functions:

  • Manages a staff of care coordinators including Registered Nurses Social Workers and Community Health Workers.

  • Responsible for hiring training and overseeing day to day activities of care coordination staff.

  • Oversees the management of care coordination by assessing the population daily assigning patients to care coordinators participating in biweekly rounds leading interdisciplinary team (nurses social work and community health workers) in planning delivery and evaluation of familycentered care coordination troubleshoot complex problems in familycentered care and participate in the development and review of care coordination protocols and procedures.

  • Maintains a work environment that encourages retention development of the team by serving as a resource for educational and practice opportunities outstanding customer services and familycentered care through leadership that establish high standards of professional behavior develops cohesive teams builds positive morale and creates an environment where LCHPCC staff can develop professionally.

  • Collaborates with the leadership team to complete and document new employee orientation attendance records staff scheduling and PTO requests.

  • Mentors the care coordinators in reviewing patient information completing health risk assessments developing care plans and helping families achieve identified goals.

  • Develops critical thinking skills in others.

  • Develops partnerships with internal Lurie Divisions and Departments and Clinically Integrated Network Practices to ensure interdepartmental collaboration for patients in the care coordination program and to overcome barriers to patient care.

  • Coordinates the development of patient clinical care guidelines and patient education materials in collaboration with the LCHPCC Medical Director.

  • Educates care coordination staff on the proper implementation of these protocols and materials.

  • Collaborates with LCHPCC Medical Director and multidisciplinary team to develop and trend performance improvement indicators based on national standards and best practice.

  • Under direction of the LCHPCC Medical Director participates in data collection and education related to CCE quality improvement initiatives.

  • Attends LCHPCC Quality Committees and other quality related meetings as assigned.

  • Actively monitors the performance of their team and takes appropriate action to address any performance deficiencies as well as to recognize outstanding performance.

  • Collaborates with Medical Director and Director of Operations to implement and manage assigned programs to address the needs of specific populations or programmatic needs of particular funding sources (grants contracts etc).

  • Other job duties as assigned.

Knowledge Skills and Abilities:

  • Masters degree in nursing or social work preferred or BSN prepared nurse with 10 years of pediatric nursing experience and 2 years experience managing other staff.

  • Certification/Licensure/Registration: Registered Nurse in the state of Illinois or License in Clinical Social Work in the state of Illinois (please attach to profile).

  • Strong organizational skills and experience planning and managing projects.

  • Familiarity with pediatrics chronic illness human development and psychosocial issues germane to chronic illness transitioning across the life span.

  • Experience in conducting interdisciplinary educational trainings.

  • Excellent leadership skills and ability to collaborate with teams.

  • Excellent presentation written and oral communication skills.

  • Proficient and knowledgeable in working with MS Office applications (Excel PowerPoint Word Outlook)

Pay Range

$89440.00$147576.00 Salary

The pay range for this job indicates minimum and maximum targets for the position.
