Job Description
We are seeking an experienced and motivated **Director of Care Management** to join our healthcare team in Fresno CA. This is a strategic leadership role that oversees care coordination discharge planning and utilization management to ensure highquality patientcentered care across the continuum. The Director of Care Management will work collaboratively with clinical teams physicians and support staff to enhance patient outcomes optimize resources and promote efficiency.
**Key Responsibilities:**
Develop implement and oversee care management processes to support quality improvement and effective patient care coordination.
Lead a team of case managers and social workers providing guidance mentorship and professional development.
Ensure compliance with all healthcare regulations accreditation standards and organizational policies.
Collaborate with clinical staff physicians and administrative teams to streamline transitions of care discharge planning and patient advocacy.
Analyze data to monitor performance metrics identify trends and drive continuous improvements in care management services.
Manage budget and resources for the care management department ensuring operational efficiency.
Foster strong communication and partnerships within and outside the organization to support patient needs and organizational goals.
**Qualifications:**
Must be a licensed **Registered Nurse (RN)** in California.
Bachelors degree in Nursing required; Masters degree in Nursing Healthcare Administration or a related field preferred.
Minimum of 5 years of experience in care management or case management with at least 2 years in a leadership role.
Strong knowledge of healthcare regulations discharge planning and utilization review.
Excellent interpersonal communication and organizational skills.
