Job Description

The Adult Social Care Review Team covers a wide remit of reviews and reassessment across the Wirral working with persons recently discharged from hospital and those residing in the community. The Review Team supports with various adult social care projects and atrisk care home and domiciliary providers.

In addition collaborating with Wirral contracts & commissioning and quality improvement teams adhering to outcomes focused and legal literate practices. The Review Team offers a plethora of opportunities for practitioners seeking to strengthen their practice and apply transferrable knowledge and skills in a CPD focused team. Social Workers are required to support with the latter service delivery utilising assessment and intervention skills framed in legal literate practices. Experience of working with older adults.

The work is diverse and the aim is to promote the wellbeing of adults with care and support needs to continuously improve their outcomes creating lasting connections with their local community.

Requirements:

  • Social Work England registration
  • Relevant social work qualification
  • Evidence of Continuous Professional Development
  • Driving licence and access to a car for work purposes

Appointment to the posts will be subject to successful interview references DBS clearance and an Occupational Health check.

This post requires a DBS check.
