The new UAB Rehabilitation Pavilion will include 78 rehabilitation beds and the latest technology all designed to promote functional activity at every turn.Our team of highly skilled therapists practice in a collaborative fastpaced environment providing comprehensive evidencebased care thats focused on each patient and their road to recovery.We support professional clinical development by empowering our clinicians through mentorship and other growth opportunities that elevate our standard of care. Job Description:
To coordinate care services conduct utilization review and provide case management for Spain Rehabilitation Center patients in a cost effective manner. To orient and support the patient and family to the rehabilitation process. To assess patients status. To coordinate and facilitate discharge planning. To effectively transition the patient through the continuum of care.
Bachelors degree from an accredited program of Nursing Occupational Therapy Physical Therapy or
