The Breast Health Navigator supports Patient Care Services (PCS) by overseeing the care of all breast cancer inpatients in Northwestern Medicines West and Northwest Regions. The Navigator works collaboratively with all members of the multidisciplinary team to establish an evidence based continuum of care plan with the goal of optimizing clinical outcomes. The main responsibilities are patient rounding review of documentation of required care processes coordination or direct provision of patient education and development of a plan with case management and our communitybased and outpatient services appropriate for level of complexity. With Medical Director establishes clinical standards order sets policies and procedures for the provision of services. The individual in this position will be responsible to identify care processes in need of improvement and proactively act to resolve unfavorable outcomes. Will serve on teams in an advisory capacity regarding updates to protocols procedures care guidelines and quality improvement.
