DescriptionThe Health Information Management Deficiency Analysis Technician who reports to the Director Health Information Management performs deficiency analysis on discharged medical records and locates incomplete/deficient dictation. Maintains refusal reports and other reports related to physician record completion and performs other job duties as assigned.
Responsibilities- Deficiency Analysis
- Performs deficiency analysis on each discharged medical record to review content of documentation to ensure completeness of legal medical record within HIM policies and procedures
- Analyzes records for authentication of documentation as required by regulatory agencies including Joint Commission and Department of Health and Hospital Services
- Locates incomplete/deficient dictation within the hospital and outsourced transcription agency dictation systems to assist with meeting established DNFB (Discharged Not Final Billed) revenue cycle goals
- Actively works various lists (i.e. Refusal List and Completed Dictation List) to assure validity of deficiencies.. Researches and troubleshoots the physician refusal report on a daily basis.

- Assists with physician training on chart completion issues.
- When requested assists with physician issues related to completion of medical record documentation
- Quality
- Adheres to established quality standards for deficiency analysis
- Strives to promote the quality and efficiency of his/her own performance through participation in staff education programs approved continuing education courses and specialized skill training programs
- Production
- Demonstrates the ability to meet the established productivity standard for deficiency analysis based on record type.

Qualifications- High School or equivalent with three years of medical records experience
