The Toward Home Alliance formed in response to South Australias 2020 homelessness reforms brings together Baptist Care SA Lutheran Care Mission Australia Sonder The Salvation Army and SAHA. United by a bold personled vision were working to make homelessness rare brief and nonreoccurring through culturally appropriate integrated and responsive services.
We are seeking a Toward Home Wellbeing Senior Practitioner to oversee daytoday team practice and ensure exceptional service delivery. This role focuses on supporting clients to achieve positive outcomes by addressing the factors contributing to homelessness and identifying sustainable housing pathways. You will also manage referrals conduct risk assessments coordinate crisis responses and lead the teams assertive outreach efforts.Guided by lived experience and a commitment to quality and continuous improvement every role in the Alliance drives meaningful systemwide change.
Experience Requirements:
- Demonstrated understanding of the homelessness sector including housing providers and referral pathways
- Demonstrated experience in engaging with homelessness community groups and individuals
- A minimum 3 years experience in the area of mental health or homelessness service delivery
- Experience in a leadership role
- Demonstrated experience working within a fastpaced environment
- Experience in working with Aboriginal communities and the ability to provide culturally appropriate support
- Demonstrated experience in working effectively with people with complex health conditions including mental health and chronic disease
- Ability to capture and share information with relevant health care providers including in electronic format
- Experience in crisis management
Qualification and Other Requirements:
- Tertiary qualifications in Social Work Social Sciences Human Services or related
- Drivers Licence
- Current or willing to obtain a Working with Childrens Check (WWCC)
