This is a full time (40 hours per week) benefited position. Employment is provided by Heluna Health
The pay rate for this role is $28.85 per hour.
ESSENTIAL FUNCTIONS
Caseload Management
- Manage and monitor a lowerrisk caseload (Tier 3) conducting outreach assessments and milestone checkins.
- Coordinate and assign appropriate roles and responsibilities within the care team to ensure efficient service delivery and client support.
- Maintain regular contact with clients to ensure engagement and adherence to care plans. Track client progress and adjust support strategies based on evolving needs.
- Participate in case reviews case conferences and coreview Comprehensive Health Assessments (CHAs) alongside the RN and Care Coordinator to inform shared care planning.
Care Coordination and Logistics
- Coordinate client appointments transportation services and access to durable medical equipment (DME) to support continuity of care and reduce barriers to service.
- Conduct proactive outreach and followup on referrals to ensure timely connection to services and sustained client engagement. Document all outreach attempts.
- Monitor logistical followthrough and resolve any barriers to care.
Interdisciplinary Support
- Provide support to Psychiatric Mental Health Registered Nurse (RN) Behavioral Health Case Managers (BH CMs) and Community Health Workers (CHWs) by assisting with delegated tasks to enhance team efficiency and client care.
- Assist in reinforcing care plans managing communications and preparing documentation
- Assign and clarify care team roles to optimize workflow and service coverage.
Case Reviews & Continuous Improvement
- Participate in regular (systematic) and asneeded (ad hoc) case reviews to support collaborative care planning and ensure quality service delivery.
- Contribute to quality improvement initiatives and team learning.
- Share client updates identify service gaps and help develop solutions that improve outcomes.
- Other duties assigned.
JOB QUALIFICATIONS
Education/Experience
- A Bachelors degree or higher from an accredited college or university in Health Information Systems Public Health Public Policy Psychology Social Work or a related field preferred
- Active Licensed Professional Clinical Counselor (LPCC) or Licensed Clinical Social Worker (LCSW) preferred.
- Experience with Case Management and certification of Case Management preferred.
- Experience with researching studying and making recommendations to support health or social service programs or policy.
- Two (2) years in a highly responsible management experience in program administration for underserved populations preferred.
- Strong organizational skills including an ability to manage multiple work projects simultaneously track project details and meet deadlines.
- Strong technical skills with Microsoft excel and experience with database management (e.g. Electronic Health Record Systems) preferred.
- Ability to attend meetings provide training technical assistance and other jobrelated duties in locations throughout Southern California and have reliable transportation to carry out essential functions.
Certificates/Licenses/Clearances
- A valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out jobrelated essential functions.
Other Skills Knowledge and Abilities
- Proficient skill set in using an array of Microsoft Office Suite software programs such as Word Excel PowerPoint Access Adobe Reader One Note Outlook Publisher Teams Outlook Zoom etc.
- Able to multitask and set workload priorities for time sensitive projects/tasks.
- Ability to problem solve and make recommendations to processes policies etc.
- Able to communicate with all levels of personnel e.g. written verbal in a professional and concise/clear manner; ability to work within a project team and/or independently.
