Description

Are you passionate about improving the quality of care for patients with chronic conditions Do you have a knack for compliance management and staff training Carolina Health Centers is looking for a dedicated Chronic Care Management (CCM) Program Coordinator to join our team and build our CCM program from the ground up!


About the Role:

As the CCM Program Coordinator you will be responsible for building and managing our CCM program. This includes ensuring compliance with Medicare regulations training staff managing workflows and monitoring program metrics and patient outcomes. Your efforts will directly contribute to enhancing the quality of care for our patients.


Key Responsibilities:

  • Program Development: Design and implement the CCM program ensuring it meets all Medicare rules and regulations. Stay updated on policy changes and implement necessary adjustments.
  • Staff Training and Workflow Management: Develop and conduct training programs for staff optimize workflows and provide ongoing support to maintain high standards of care.
  • Program Monitoring and Evaluation: Monitor key program metrics and patient outcomes analyze data to identify areas for improvement and prepare regular reports for the Director of Quality and Population Health.
Requirements

Requirements:


Education:

  • Bachelors degree in healthcare administration nursing or a related field.

Experience:

  • Minimum of 3 years in healthcare program management preferably in chronic care management. Strong knowledge of Medicare rules and regulations.

Skills:

  • Excellent organizational communication and leadership skills. Proficiency in data analysis and reporting. Ability to work collaboratively with healthcare professionals and staff. EPIC experience preferred.

Licensure:

  • RN preferred

Physical Abilities:

  • Manual dexterity for operating medical equipment computer and other office tools. Vision abilities required include close vision distance vision color vision peripheral vision depth perception and ability to adjust focus.


Work Environment:

  • Officebased with some remote work flexibility.
  • Occasional travel to healthcare facilities.

Reporting Relationships:

  • Reports to: Director of Quality and Population Health
  • Supervises: Care Managers
