You will be updated with latest job alerts via emailTo work directly with clients of the Assertive Community Treatment Team (ACT) by using the evidence base practice model of ACT designed by NAMI to provide comprehensive and wrap around services to address client goals of mental health physical health housing employment social goals and other goals as identified by the client. To utilize clinical skills and experience to assess plan develop coordinate and provide treatment rehabilitation and support services to program clients with severe and persistent mental illnesses under the clinical supervision of the ACT Manager and ACT psychiatrist. To function as a clinical member of the multidisciplinary team and provide treatment rehabilitation and support services or may be designated by the ACT Manager to function as the lead mental health professional team vocational specialist or team substance use specialist. To provide mental health supports to clients of the ACT team as well as assist in educating and coordinating with team members to meet client mental health needs.

Client Service:
Conduct comprehensive assessment of psychiatric history mental status and diagnosis; use of drugs or alcohol; education and employment; social development and functioning; activities of daily living; and family structure and relationships.
Provide service coordination (case management) for an assigned group of clients including coordinating and monitoring the activities of the individual treatment team; assume primary responsibility for developing writing implementing evaluating and revising overall treatment goals and plans in collaboration with the client and ITT; provide individual supportive therapy psychotherapy and symptom management ensuring immediate changes are made in the treatment plans as clients needs change; educate and support clients families and advocate for clients rights and preferences.
Act as a liaison and consult with community agencies and families to maintain coordination in the treatment process.
Perform shift management in coordination with other ACT shift managers according to established policies and procedures.
Provide oncall crisis intervention covering nighttime hours and serve as primary team member during evenings and weekend shifts as assigned.
Provide treatment rehabilitation and support services with some interventions directed or performed by staff with specialty training and skills (e.g. vocational sepcialists).
Take the lead role or participate in providing substance use and treatment services.
Provide direct clinical services to clients on an individual group and family basis in the office and in community settings to teach behavioral symptommanagement techniques to alleviate and manage symptoms not reduced with medication and to promote personal growth and development by assisting clients to adapt to and cope with internal and external stressors.
Provide multiplestage individual and group treatment I the office and in community settings to develop a trusting relationship with clients so that they may more freely discuss substance use and its effect on mental and physical health and daily functioning. May participate in setting treatment goals and plans with clients to reduce or abstain from substance use and to learn replacement behaviors.
Coordinate with outside inpatient services to detoxify clients and establish linkage to outpatient treatment selfhelp (e.g. Alcoholics Anonymous Narcotics Anonymous etc.) outpatient services and residential facilities.
Assess the needs of clients and develop goals that will allow them to achieve the greatest level of independence in their community.
Provide skills teaching to clients and work with them and other team members to creatively explore alternatives to hospitalization higher level of care homelessness and incarceration.
Advocate for clients to help meet their needs desires and best interests.
Utilize critical thinking in making independent judgements and maintain responsibility and accountability for service provided.
Knowledge of the effects of mental illness and the ability to relate respectfully to clients with a wide range of abilities and diagnoses.
Ability to meet clients in their homes or in public places with ability to drive safely and a willingness to transport clients in own vehicle.
Possess teamwork skills including problem solving cooperation negotiating etc.
Ability to receive medication management certificate within first 3 months of hire to be able to administer medications to clients in their homes and in the community while keeping medication manager certificate for duration of employment.
Ability to be available for oncall shifts for evenings and weekends which includes carrying the 24 hours oncall client crisis line and delivering medications as well as providing crisis triage services via phone an potential on scene.

Documentation:
Document client progress to maintain a permanent record of client activity within established guidelines.
Complete all documentation within established guidelines.
Ability to prioritize client need and to complete required written documentation.
Demonstrate an understanding of all funder and accreditation standards.
Track and document monthly outcome objectives.
Education: Masters Degree Required in Social Work Mental Health Counseling Psychiatric Rehabilitation Psychology or other human services related field.
Experience: One year working in Mental Health or Behavioral Health.
License(s)/Certification(s): Valid drivers license when driving any vehicle for workrelated reasons. License as applicable to masters level degree. Ability to obtain/maintain medication management certificate within 3 months of hire. Certified Alcohol and Drug Abuse Counselor in good standing.
Knowledge/Skills/Abilities: Ability to speak read and write fluent English. Ability to read and comprehend common terminology of the mental health field. Ability to enter documentation in an EHR and use Microsoft Office. Ability to effectively present information and respond to questions from clients medical professionals management staff and the public. Ability to assess diagnose and work on identified symptoms. Knowledge and understanding of the Assertive Community Treatment Model.
