Job Description

Overview

Select Hospice is driven by the philosophy of commitment to our patients being renowned for our excellence in patient care and dedicated leadership team as well as care team.

The Hospice Certified Nursing Assistant (CNA) provides supportive and personal services for the patients with dignity and privacy. Assist with personal care and hygiene to hospice patients.

Responsibilities

The person in this position must be able to perform the following essential job functions with or without reasonable accommodations.

1. Follow the instructions of the professional nurse/therapist in providing care.

2. Assist with hygiene such as bathing oral care and dressing.

3. Perform and record accurate measurements (i.e. vital signs or intake/output as instructed in the care plans).

4. Observe and report any safety hazards found in the clients home or any significant observations regarding the client.

5. Attend staff meetings attend/complete minimum state and federal standards of annual in services and participates in orientation of new employees.

6. Report patient complaints to the RN.

7. Maintain patient confidentiality and adheres to HIPPA requirements and agency Policy and Procedure manual.

8. Possess the ability to follow written and oral instructions.

9. Report abnormal findings in patients conditions as observed and per RN care plan.

10. Complete assigned trainings on time.

11. Performs other duties as assigned.

12. Perform the following task when delegated by the RN:

Noninvasive and nonsterile treatments

Collection and documentation of vital signs height weight I/O environmental situations and client behaviors

Ambulation positioning and turning

Transportation

Personal hygiene and elimination (irritations enemas)

Feeding

Socialization

ADLS

Health teaching reinforcement that has been planned and reinforced by the RN

Qualifications

1. Graduate of an accredited High School or equivalent preferred.

2. Ability to read write and follow directions.

3. Must be a certified nursing assistant with proof of certification OR a certified home health aide

4. A minimum of oneyear fulltime experience in direct patient care in an institutional setting (hospital or nursing facility) or oneyear fulltime experience within the last five years in direct client care in an agency setting.

5. Work positively and favorably with patients families and staff.

6. Demonstrate compassion responsibility and cheerful attitude.

7. Listed on the nurse aide registry with no finding against the aide relating to client abuse neglect or misappropriation of client property.
