Intensive Care Coordinator

Location: 241 Greenhouse Road Lexington VA
Employer: Rockbridge Area Community Services (RACS)

ROCKBRIDGE AREA COMMUNITY SERVICES RACS is currently seeking a motivated qualified individual to work in our coverage area of Rockbridge and Bath County and the Cities of Lexington and Buena Vista. RACS will provide training supervision and education opportunities specific to the position.

Position Description:

The Intensive Care Coordinator (ICC) is a vital member of the Child & Adolescent CommunityBased Services team responsible for providing intensive care coordination to children and adolescents with emotional and behavioral challenges. This role supports youth who are transitioning from residential care or are at risk of outofhome placement by helping stabilize them within their home and community.

The ICC works closely with families service providers and community partners to develop individualized care plans rooted in family strengths needs and culture. The goal is to build a strong wraparound system of support that promotes longterm success. This position is ideal for a teamoriented professional with strong communication skills clinical insight and a passion for making a difference in the lives of youth and families.

Essential Work Performance Areas:

  • Communicates effectively both orally and in writing.
  • Demonstrates ability to problem solve and address routine issues.
  • Works well as part of a team e.g. respects others opinions shares workload as appropriate and demonstrates flexibility.
  • Projects positive attitude toward clients staff and other agencies; treats clients staff and other agencies in a caring and respectful fashion; mindful of individual cultural and ethnic differences.
  • Maintains appropriate professional boundaries (e.g. with clients other staff agencies etc.).
  • Sets appropriate priorities plans work systematically uses time wisely.
  • Enthusiastic motivated takes initiative.
  • Actively participates in supervision; utilizes supervision appropriately; is responsive to constructive feedback; consults with supervisor as necessary when crises or situations beyond the norm arise.
  • Demonstrates openness and willingness to change; initiates change when necessary; encourages others to accept change.

Qualifications

Education & Training Requirements:

  • Must meet qualifications as a Qualified Mental Health Professional Child (QMHPC) at the time of hire
  • Bachelors degree required
  • Minimum two years of experience working with children/adolescents in a clinical or case management role
  • Must obtain CANS certification within 90 days of employment
  • Completion of HighFidelity Wraparound training as soon as available after hire
  • Completion of all Child Mental Health Case Management and Prescreener certification modules within 6 months of employment

Background Investigation Requirements: All applicants are subject to a background investigation. Investigations may include fingerprint checks (State Police); local agency checks; employment verification; verification of education (relevant to employment); and other checks based on the position.

Probationary Period: All persons that are newly hired rehired transferred promoted or demoted into a Rockbridge Area Community Services position must serve a new 6month probationary period.
