Cornerstone Family Healthcare is actively recruiting for a Substance Abuse Counselor to join our growing team in Newburgh.
RATE OF PAY/SALARY: $24.73$32.97 and sign on bonus
WORK LOCATION(S): 3 Commercial Pl Newburgh NY 12550

Description of Duties:
- Provide case management and advocacy services to an assigned caseload.
- Participate in monitoring the therapeutic environment to ensure that the behavioral guidelines are followed a therapeutic environment is maintained and interpersonal relationships remain positive.
- Provide psychoeducation and counseling appropriate to chemical dependency and relapse prevention.
- Participate in assessment screenings to determine client needs/preferences; formulates and implements treatment transition and discharge /continuing care planning collaboratively with individual clients.
- Facilitate the resocialization process of clients by serving as a role model. Provides role modeling regarding ethical and professional conduct.
- Conduct facility safety checks as assigned to visually assess clients for the use of alcohol/drugs or the environment of care for unsafe conditions.
- Ensure the maintenance of accurate complete timely and highquality client records that comply with external regulatory standards and agency policy and procedure.
- Comply with 42 CFR confidentiality and HIPAA privacy and security regulations.
- Work cooperatively with other staff as a member of the interdisciplinary team.
- Function as a liaison/advocate to clients families and other agencies as needed.
- Provide crisis intervention generating referrals to agency or external mental health providers or other service providers as indicated.
- Attend and participate in weekly department monthly staff and other required meetings.
- Monitoring of supervised toxicology screens as needed
- Assist in peer/client chart audit reviews.
- Participate in counselor of the day (COD) responsibilities.
- Be familiar with and adhere to all clinical administrative and general policies and procedures as outlined in the Cornerstone Family Healthcare Policy and Procedure Manual and Employee Handbook.
- Maintain confidentiality of all aspects of Cornerstone Family Healthcare including but not limited to patient confidentiality financials and employee relations.
- Perform other duties as assigned.
- CASAC or QHP
- Bachelors or Masters is preferred
- Computer skills
- Spanish speaking preferred
