Job Description
A facility located east of Louisville KY is seeking a Director of Case Management to join their team!
Pay: $110000 $135000 / annually
Director of Case Management Opportunity:
- Fulltime directhire opportunity
- Schedule: varies mostly Monday Friday
- Setting: Hospital
Director of Case Management Responsibilities:
- Assume responsibility for the daytoday operations and human resource management of the Case Management department
- Director of Case Management will provide guidance and support to Case Managers and other staff including training on managing caseloads and interpreting regulations policies operational procedures and objectives. Review operations to ensure a high level of quality consistent with organizational standards
Director of Case Management Requirements:
- Must possess a minimum of a bachelors degree
- Current CCM or ACMTM certification or willing to obtain within one year
- Must be an RN OT PT ST or Rehab Counselor
- 3 years of hospital based case management experience
- 3 years of utilization Review and discharge planning experience
