Fort Dodge
UnityPoint Health Fort Dodge provides coordinated clinic hospital and homebased care for patients in Fort Dodge and surrounding communities. As a designated regional referral center Trinity Regional Medical Center offers a wide range of primary and secondary care services. Trinity is accredited by DNV Healthcare Inc. and has achieved Pathway to Excellence designation by the American Nurses Credentialing Center.
General
- Established team of 2 physicians and 4 Advanced Practitioners
- Opportunity to practice outpatient psychiatric medicine in a Certified Community Behavioral Health Clinic CARF accredited belonging to the Iowa Behavioral Health Association
- Onsite pharmacy lab and family medicine clinic
- Cohesive group of colleagues and supportive staff allowing for autonomy while building the practice according to your preferences
- Experienced staff both clinical and nonclinical who know the patients and the community
- Fulltime position including dedicated administrative time with flexible scheduling to ensure sufficient patient access
- Fully integrated system with clinics and hospitals on EPIC EMR to improve care coordination
Responsibilities
- Average of 1520 patients per day; no nights holidays weekends
- Flexibility within schedule Monday through Friday
- No call requirement
- Practice is a mix of outpatient family medicine and mental health
- Willing to see children and adolescent patients
- Ability to work as a team member to improve the health of the people and communities we serve
- Specialty skills: history of working with substance abuse disorders behavioral health screenings substance abuse screenings social determinants of health etc.
- Evaluates interprets and coordinates information to make decisions and recommendations regarding patient care
- Patientfocused friendly and willing to collaborate in a team environment
- Excellent communication (written and verbal) analytical and problemsolving skills
Qualifications
Advanced Practitioners
- Nurse Practitioner PMHNP
- Ideally Board Certified with Family Medicine and Psychiatry
- Experience preferred but not required
- Hold an unrestricted medical license in the state of Iowa
- Hold current and unrestricted state and federal authority to prescribe and administer medications as necessary or appropriate to the Advanced Practitioners field of practice
- Eligible to work in the United States
