Our pediatric specialty clinic is looking for an RN navigator to join the team! This role will help coordinate care to ensure families and patients have the treatment education and resources necessary to meet their needs within the Pediatric Nephrology department. This department treats patients with chronic kidney disease congenital renal anomalies glomerulonephritis and a variety of other nephrology related concerns. In addition our staff along with our pediatric nephrologist go to several outreach clinics throughout the region allowing for increased access to the specialized care that is essential to their health and wellness. Collaborates with healthcare team and other professionals to coordinate and ensure timely sequencing of care. Integration enhances patient flow and interdisciplinary communication promotes early intervention. Supports patients/clients and other family members by removing barriers of care.
Seeks to evaluate outcomes based on an integration among established clinical financial and utilization data. Functions in structured and unstructured health care settings described as a geographic and/or situational environment that may not have established policies and procedures. Utilizes independent nursing judgment when integrating health care. Knowledge of and utilizes appropriate agerelated patient care protocols relating to the physical and psychological needs of pediatric adolescent adult and geriatric patients according to department scope of service.
May serve as a key link between providers clinics insurance concerns communication inpatient floors and outside facilities. Establishes and maintains good interpersonal relationships with those with whom has contact. Able to work with growth and development needs of client populations in clinical area. Strong communication skills are required. Coordinates facilitates and negotiates with others. Monitors evaluates and trends patient responses utilizing structured care methodologies. Work closely with ancillary and support services. Ability to solve problems identify and implement process improvements when needed. Selfmotivated working with little direction. Ability to facilitate groups multidisciplinary teams and implement best practices when necessary.
Functions within the scopes and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in state of practice and licensure. The Sanford Professional Nursing Practice recognizes the Scope and Standards of Practice and the Code of Ethics for Nurses with Interpretive Statements as published by the American Nurses Association as the foundation of nursing care delivery and professional conduct.
Qualifications
Graduate from a nationally accredited nursing program preferred including but not limited to Commission on Collegiate Nursing Education (CCNE) Accreditation Commission for Education in Nursing (ACEN) and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA). Bachelors degree in nursing preferred.
Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state. Obtains and subsequently maintains required department specific competencies and certifications.
