Through a multidisciplinary approach integrating medical nursing and rehabilitative services they deliver comprehensive patientcentered care.
Key Responsibilities:
- Conduct home visits to provide psychoeducation support and care coordination for clients and caregivers
- Assess patients medical psychological social environmental and vocational needs
- Collaborate with Allied Health Professionals doctors and nurses to ensure holistic care
- Offer psychological expertise in multidisciplinary meetings
- Provide training for team members and caregivers
- Facilitate Mental Health support groups for patients and caregivers
- Execute projects and responsibilities assigned by the supervisor or Head of Department
Requirements:
- Registered Nurse with the Singapore Nursing Board
- RMN/Advanced Diploma in Mental Health or relevant qualifications (e.g. social work counseling psychology)
- 3 to 5 years of psychiatric experience (Community psychiatric clinic/IMH preferred)
- Skilled in psychotherapy for mental health patients
- Strong organizational interpersonal communication and writing skills
- Proficient in MS Office and familiar with healthcare environments
- Ability to multitask work independently and collaborate effectively
- Passionate about supporting individuals with mental health disorders
