Primary Purpose
Responsible for enhancing the delivery of evidencebased medicine through designing leading implementing and monitoring of medication use systems and programs. Assists with the strategic planning and execution of pharmacy initiatives serving as a subject matter expert.
Major Responsibilities
Minimum Job Requirements
Education: Bachelors or Doctorate degree in pharmacy from an ACPEaccredited school of pharmacy required or an FPGEC certificate from NABP
Certification / Registration / License: Licensed in the state(s) of practice or eligible for reciprocity required. Immunization and nonvaccine injectable certification required based on area of practice. Credentialing and privileging required based on area of practice.
Work Experience: Requires completion of pharmacy residency training or equivalent experience or working experience as a licensed pharmacist in area of specialty. Direct experience with training education or preceptor roles required. 3 years of hospital pharmacist experience.
