Job Description
The RN Case Manager position will be responsible for the following duties at Adena Pike: Navigating the swingbed needs/discharge planning Facilitates transition planning across the care continuum by involing the patient family providor and other healthcae team members. Works with patients/families to promote self management support of chronic diseases through individualized plans with short/long term goals. Ensures suspected cases of abuse neglect or exploitation have been referred/reported to the appropriate parties or agencies. Required Certification: BLS Required Experience: RN Case Manager
Shift: Days 8am 430pm
