About the Position:
Note: This position requires reporting to the assigned duty location; no remote option will be available.
This position exists to assist the unit MSW with overseeing the plan of care process and other admission discharge and treatment responsibilities. The Health Professional III attends all POC process meetings listens to the clinical discussion and documents the discussion in the review summary section in the POC form. The Health Professional III must be familiar with the plan of care policies and the regulatory standards they are based upon (CMS CDPHE DMH JCAHO etc.) The duties include: training clinical staff on the POC process documentation requirements and BEHR entry for the POC; auditing POCs for accurate and timely completion; producing audit reports that show trends in errors or staff deficiencies and include correction plans; assisting on back up group therapy coverage; and assisting in admission and discharge needs including shopping for patients transports for placement or home visits and miscellaneous unit duties as assigned.
Completes audits under the direction and in collaboration with the Social Work team. Collects and summarizes findings. Reports findings to the treatment team and SW Supervisor. Assists with completing data input and collaborates on system maintenance for SW audits focused audits or focused audits as requested/required. Assists in determining the best method of data presentation based on the needs of the team. Uses an established database in which results of all audits are maintained and through which comprehensive reports (including graphs and charts) may be extracted for the Director of Social Work QSS Executive Committee or Program and Department Heads. Other duties include:
Assisting with plans of care and followup documentation
Any other duties that may arise that need to be completed by this position that are not covered in the above duty statements.
Six (6) yearsof relevant experience in Social Work or an occupation related to the work assigned to
this position
OR
A combination of related education and/or relevant experience in an occupation
related to the work assigned to this position equal tosix (6) years
Preferred Qualifications:
Knowledge of the principles of human growth and behavior basic sociologicaland psychological treatment and therapy practices
Knowledge of interviewing skills and techniques
Ability to express ideas effectively orally and in writing
Ability to use interviewing skills and techniques effectively
Ability to exercise good judgment in evaluating situations and making decisions
