CMHHIP units provide comprehensive psychiatric and psychosocial evaluation and treatment by a multidisciplinary treatment team. This includes: 24 hour nursing care and assessment; psychiatric intervention and evaluation; psychological services; psychosocial services; occupational therapy educational and vocational rehabilitation services and recreational therapy. Patients are admitted from throughout the state from correctional court mental health and other community facilities for long and short term treatment under court authority as well as transfers from within CMHHIP and other units.

Description of Job

About the Position:
Note: This position requires reporting to the assigned duty location; no remote option will be available.
This position exists to assist the unit MSW with overseeing the plan of care process and other admission discharge and treatment responsibilities. The Health Professional III attends all POC process meetings listens to the clinical discussion and documents the discussion in the review summary section in the POC form. The Health Professional III must be familiar with the plan of care policies and the regulatory standards they are based upon (CMS CDPHE DMH JCAHO etc.) The duties include: training clinical staff on the POC process documentation requirements and BEHR entry for the POC; auditing POCs for accurate and timely completion; producing audit reports that show trends in errors or staff deficiencies and include correction plans; assisting on back up group therapy coverage; and assisting in admission and discharge needs including shopping for patients transports for placement or home visits and miscellaneous unit duties as assigned.

Completes audits under the direction and in collaboration with the Social Work team. Collects and summarizes findings. Reports findings to the treatment team and SW Supervisor. Assists with completing data input and collaborates on system maintenance for SW audits focused audits or focused audits as requested/required. Assists in determining the best method of data presentation based on the needs of the team. Uses an established database in which results of all audits are maintained and through which comprehensive reports (including graphs and charts) may be extracted for the Director of Social Work QSS Executive Committee or Program and Department Heads. Other duties include:

Assisting with plans of care and followup documentation

Continuity of Care
  • Works under the direction of the unit master level social worker (MSW) and under the direction of the Social Work manager assists in implementing patient care and discharge needs.
  • Restraining order process which includes receiving an email from Legal Records with the victim information The unit Mental Health Liaison logs and/or retains this information.
  • Responsible for ensuring that victims are not listed on the patients approved visitor list. Within the first 48 hours the Mental Health Liaison reviews the restraining orders from the courts through the Medical Records Legal Department.
  • The restricted persons must be listed as No Contact. For ongoing calls the Bachelor level Social Worker develops an approved call list for patients with restraining/protection orders as individually indicated for staff to reference.
  • The BSW works with the patient to schedule visits from family friends and support persons and submits this information to the unit Social Worker for approval. Other duties assigned by the SW manager.
Program Participation
  • Provides backup program coverage as assigned by the MSW.
  • Cofacilitates psychosocial therapy groups and assesses patient education needs and identifies readiness to learn motivation strengths and barriers to the learning process.
  • Provide ongoing psycho/social education that is relevant to individual social environmental factors medical regimens to enhance mental health growth and stability.
  • Educate patients regarding related symptoms of their disorders symptom management risk factors relapse prevention and level of care needs.
  • Documentation of above.
Crisis Management
  • Participates in milieu management by developing a rapport with the patients on the unit.
  • Implements methods that offer immediate short term help to the individual(s) that are experiencing an event that is producing emotional mental and behavioral distress or problems.
Treatment Planning Coordination
  • Keeps the schedule of the treatment planning sessions meetings morning reports and other meetings as directed.

A combination of related education and/or relevant experience in an occupation

Absent extraordinary circumstances prior disciplinary termination or resignation in lieu of termination will disqualify the applicant from future State employment with CDHS
  • Up todate on MANDT and CPR prior to working first assignment
  • Able to: Lift arms above head and kick as high as own waist
  • Stabilize another person to accomplish a controlled take down
  • Rotate body 90 degrees with feet planted for using a defensive tactic
  • Kneel on one or both knees and stand up again with multiple repetitions
  • Must be able to see and hear in order to observe address and respond to potential breaches in safety
  • Seize hold control or subdue violent or assaultive persons and defend oneself or others to prevent injury
  • Demonstrate body agility strength of all four limbs and the ability to walk and climb stairs consistently
  • Work in situations involving assaultive behavior physical control of another person and/or restraint situations.
  • Shall not initiate or engage in sexual social personal or financial relationships with patients either during or any time after hospitalization even if the patient requests or initiates such a relationship. All relationships with current or former patients must be disclosed on your application
  • Successfully pass all modules of the CMHHIP New Employee Orientation core curriculum to include English reading comprehension relevant competency assessments and written/practical exams
  • Has direct patient care responsibilities; work in situations involving assaultive behavior
  • Work in an environment with fluctuating noise and temperature (hot and cold) which may include working outdoors in inclement weather and/or concrete floor/wall work settings
  • Ability to read and comprehend the English language; effectively and fluently communicates in English both verbally and in writing
  • 2step TB (tuberculosis) screening is required of all new employees upon hire
  • Annual influenza and other vaccinations as required per the State of Colorado
  • Willing and able to travel

Appeal Rights:
