As we continue to grow our team we are looking for a Case Manager to join our passionate dedicated and BRAVE team! If you are a collaborative innovative individual who is interested in helping people recover from a mental health barrier increase their independence and find housing in York Region and South Simcoe than this role is for you!

Position One Contract position until March 31 2026 FullTime 35 hours per week; evening hours and weekends may be required.
Salary: $56444$60461 per annum.*This represents the first three steps of a sevenstep salary band. Candidate experience to be considered when deciding starting salary.

The successful candidate will work in a hybrid model and will be required to work out of the Markham Office located at 3601 Highway 7 East HSBC Tower Suite 710 Markham ON L3R 0M3 CAN a minimum of 2 days per week and will be required to travel extensively within York Region & South Simcoe for client meetings staff training and meetings.
Job Profile:
The Case Manager works with clients 16 years of age and older who are experiencing significant difficulties in daytoday and role functioning as a result of serious and persistent mental illness and/or addictions. The Case Manager collaborates with the client to identify strengths and needs and develop longterm goals shortterm objectives to meet these goals and concrete action steps to achieve the objectives. As part of this process individuals are assisted to acquire and use skills and to connect with formal and informal supports peer support professional services and resources to promote their recovery improve their quality of life and reduce the likelihood of relapse. Other duties as required.
The Case Manager collaborates with clients to provide individualized and flexible services to engage support and integrate clients into the community and facilitate their use of resources and support to maximize their independence and potential. The Case Manager adheres to the agencys Health & Safety Program and the Client Safety Framework in the promotion of client safety. The Case Manager participates in individual and systemic advocacy initiatives to improve resource access and availability.
In addition to compliance with all organizational policies and procedures the incumbent is also expected to behave ethically demonstrate competence in effective communication and team building work in a collaborative environment and have confidence in decisionmaking and building relationships.
The Case Manager may be assigned to different programs as required based on need (i.e.: Flexible Support ACTT Case Management etc)
