McMan is recruiting for a Case Manager to join our Hope Homes program. Operating from a Housing First and Recovery Oriented System Approach the Case Manager will be responsible for assisting youth in obtaining and maintaining safe housing supporting their education or employment goals and any other areas of support that is required to help them successfully transition out of homelessness and into independence.
We are looking for someone who has a passion for working with youth who are experiencing or at risk of becoming homeless and one who shares McMans core values of Respect Trust Empathy Genuineness and Commitment.
Some of the key aspects of this role include:
- Managing an assigned case load of 8 youth.
- Meeting youth program and organizational needs while working from a flexible schedule that could include evenings.
- Completing a variety of assessments with youth and documentation surrounding casework and goal planning.
- Assisting youth in developing and implementing Individualized goal plans.
- Providing comprehensive community resource information to youth based on individual needs.
- Educating and supporting youth to increase skills that promote greater selfefficacy.
- Meeting with the youth in their home or community environment.
- Providing individual instruction modeling and support of basic life skills ( health nutrition home cleanliness safety).
- Providing support in the areas of addition mental health and education or employment.
- Monitoring and supporting youth through a Recovery Oriented System Approach to ensure living conditions are safe and sanitary.
- Supporting the organization by participating in a 7 day Oncall rotation every 1012 months.
The successful candidate will have:
- A degree or diploma in Human Services.
- Knowledge of youth homelessness community resources.
- Experience with HMIS data would be preferred.
- Three (3) years experience working with youth in case management.
- Excellent organizational and communication skills.
- Strong writing organizational program planning and interpersonal skills
- Demonstrated interest and knowledge of Indigenous history language and culture would be an asset.
- Knowledge and experience with the Relational Theory harm reduction and stages of change is an asset.
- Current standard First Aid Certification and Suicide Intervention Training.
- Valid Drivers license and adequate insurance coverage.
- The successful candidate must also have a Child Welfare Intervention Check and Police Check (current within 6 months).
*This role will require on call support on a rotating basis and some evening or weekend work hours.

Salary Range:$49768 $54931 per annum
