Description

Hennepin Health is seeking a Senior Social Worker to join the Special Needs BasicCare (SNBC) team to provide care coordination for members who are certified disabled and have complex social needs. This critical role will engage directly with members to offer care coordination and support including addressing systemic barriers and facilitating other supportive services as applicable.

This position will work within an interdisciplinary team of nurses and community health specialists to sensitively address the social determinants of health of SNBC members. This role will focus on a vulnerable population struggling with poverty and multifaceted medical issues and work to address their needs in a trauma informed and membercentric way. The person filling this role must be flexible innovative and understand marginalized populations specifically the unique health conditions affecting them and the social context in which they develop.

Location and hours:
This position is hybrid and will be performed both onsite at the Health Services Building 525 Portland Avenue South 8th floor Minneapolis MN 55415 in the field and communitybased settings and remote as job duties require. Work hours will be Monday through Friday with typical hours of 8 a.m. to 4:30 p.m. While this position is designated as hybrid based on current requirements hires must reside in or relocate to Minnesota or Wisconsin.

In this position you will:
  • Facilitate annual completion of the health risk assessment evaluate member needs; develop customized care plan reassessments; provide appropriate follow up to members. Conduct this work in the field community settings and telephonically to accommodate member needs.
  • Manage caseload of SNBC members with varied disabilities and behavioral health needs; providing responsive follow up and management of individualized care plan; work to address identified barriers conduct quarterly checkins with members.
  • Work within MnChoices and Health Edges Guiding Care (care management system) to process assessments care plans and other relevant care coordination forms; prepare and maintain records including operational regulatory and utilization reports adhering to all contractual requirements specified by the Minnesota Department of Human Services (DHS).
  • Maintain working knowledge of behavioral health resources including specialty services and programs that effectively address members needs related to housing employment education and economic status.
  • Participate in team member and all staff meetings; establish and maintain effective relationships with other case management staff to effectively coordinate member care.

  • Experience:
    • Managing a caseload within a team providing case management or care coordination services in a clinical or communitybased setting.
    • Navigating social services and housing options available to residents of Hennepin County.
  • Knowledge of:
    • Principles and practices of care coordination; service referral within a county system.
    • Information systems relevant to care management coordination and reporting.
    • Trauma informed and harm reduction approaches to care.
    • The unique challenges experienced by those impacted by mental health and chemical health problems as well as the impact of complex trauma history.
  • Ability to:
    • Work independently in various settings supporting individuals who are experiencing marginalization and distress.
    • Act in an advocacy role on behalf of member.
    • Function well in unpredictable and challenging situations calling for sensitivity flexibility resourcefulness and independent judgement.
