Come join our team and make a difference while enjoying a great quality of life!General Position SummaryThe Services Coordinator Lead provides targeted case management services to individuals with Intellectual/Developmental Disabilities (I/DD) and monitors community providers supporting individuals served by Klamath County Developmental Disabilities Services (KCDDS).
This position plays a vital role in improving service delivery supporting peers and maintaining compliance with Medicaid and Oregon Administrative Rules (OAR).Essential FunctionsThe following may not be allinclusive and other duties may be assigned.- Develop and revise ISPs or Annual Plansbased on assessments to address each individuals medical social educational and employment needs.
- Assist individuals and families in creating and implementing personalized support systems.
- Use choice advising to support individuals in selecting service providers settings and supports.
- Monitor Medicaidauthorized services and expenditures through case management contacts.
- Deliver personcentered services that encourage selfdetermination.
- Authorize and coordinate developmental disabilities services as selected by individuals or their representatives.
- Coordinate services with state public and private agencies.
- Maintain uptodate knowledge of relevant laws rules policies and procedures.
- Consistently document case management contacts and activities with weekly progress notes.
- Attend supervision meetings and participate in team training and consultation.
- Attend IEP and transition planning meetings.
- Ensure service delivery compliance with OAR Chapter 411 Division 415.
- Identify and report gaps in service delivery resources.
- Complete a minimum of 20 hours of ID/DD training annually.
- Build and maintain professional relationships with community partners and stakeholders.
- Uphold professional boundaries and confidentiality at all times.
- Fulfill mandatory abuse reporting responsibilities.
- Actively promote the mission and core values of KCDDS.
- Collaborate with SC Leadership to review and improve case management tools and materials
- Obtain SC ONA permissions from eXPRS to support ONA completions.
- Support SC caseloads during extended staff leave.
- Provide peer support shadowing opportunities and mentorship to new SCs.
- Serve as the initial contact for choice counseling for eligible individuals.
- Share feedback and identify service delivery trends or gaps with SC Leadership.
- Meet regularly with SC Leadership to ensure consistent application of case management practices.
Supervisory ResponsibilitiesThis position has no supervisory responsibilities.QualificationsEducation Certification & Experience:- Must have knowledge of Oregons public service system for developmental disabilities.
- Must meet one of the following criteria:
- Bachelors degree in behavioral science social science or closely related field and one year of developmental disabilities case management experience; OR
- Bachelors degree in any field and two years of developmental disabilities case management experience; OR
- Associates degree in behavioral science social science or closely related field and three years of developmental disabilities case management experience; OR
- Four years of developmental disabilities case management experience without a degree.
- Must possess a valid Oregon Drivers License.
