Job Description:

Your Role as a Hospice LVN

Branch: Terre Haute IN Schedule: 23 days a week Part Time

As a Hospice LVN youll provide compassionate care while meeting the unique needs of each patient. Your responsibilities include:

• Delivering Nursing Care: Provide highquality handson nursing care to hospice patients ensuring comfort and dignity at all times.
• Monitoring Patient Conditions: Observe and report changes in patients health to the interdisciplinary care team.
• Collaborating with the Team: Work closely with physicians Registered Nurses and other caregivers to implement individualized care plans.
• Educating Families: Offer guidance and support to patients loved ones ensuring they understand the care process and feel supported.

What Were Looking For

To join our team youll need:

• Licensure: Current LVN license in good standing (as required by your state).
• Graduate of an accredited Certificate Diploma Associate or Baccalaureate School of Nursing. OR Graduate of an accredited Certificate Diploma.
• Experience: At least 1 year of experience in acute care or a comparable setting.
• CPR Certification: Valid certification from the American Heart Association or American Red Cross.
• Reliable Transportation: Ability to travel to perform job duties.
• Exceptional Communication Skills: Ability to communicate effectively with patients families and colleagues.
• Positive Attitude: A dedication to providing compassionate care and exceeding expectations.
• Ability to sit stand bend lift and move intermittently with the capability to lift 50100 lbs.
