Job Description

Summary Statement

This adolescent treatment services position sits within the Child and Family Care Coordination Unit andwill be a member of the units team. Care Coordinators support children and families with carecoordination and link families to resources and services that are clinically necessary in a leastrestrictive environment to promote positive emotional and behavioral adjustment. This position worksclosely with the familys formal (e.g. child welfare juvenile justice schools etc.) and informal supports(e.g. neighbors family members clergy etc.) to improve the academic social emotional andbehavioral outcomes for their child(ren).

Essential Functions

Essential functions are fundamental core functions common to all positions in the class series and are not intended to be an exhaustive list of all job duties for any one position in the class specifications are descriptive and not restrictive incumbents can complete job duties of similar kind not specifically listed here.
  • Participates in treatment planning and discharge planning in conjunction with the facility personnel as well as with members of other involved agencies to insure that the treatment results in the expeditious release of the adolescent patient.
  • Designs the aftercare treatment plans and referring clients to appropriate outpatient treatment facilities community support systems and/or residential treatment facilities and insures that the discharge recommendations and aftercare plans are carried out.
  • Interacts with clients families interviewing members observing family dynamics assessing strengths or dysfunctions and adapts aftercare treatment plan to reflect their needs as well as those of the client.
  • Provides direct supportive services including home visits individual and family counseling as well as family crisis intervention.
  • Monitors the outpatient treatment components to insure progress and resolve any problems that might arise.
  • Develops and maintains a bank of community resources to provide the necessary services for the adolescent and family.
  • Prepares monthly statistical narrative and court reports as required detailing caseload activities.
  • Prepares progress notes on each in staff meetings.
  • Makes formal presentations before community groups schools and/or professional organizations.

Job Requirements

JOB REQUIREMENTS for Adolescent Treatment Services Coordinator
Applicants must have education training and/or experience demonstrating competence in each of the following areas:

  1. Possession of a Bachelors degree or higher in Behavioral or Social Science or related field.
  2. Six months experience in case management which includes assessing planning developing implementing monitoring and evaluating options and services to meet an individuals human service needs.
  3. Six months experience in making recommendations as part of a clients service plan such as clinical treatment counseling or determining eligibility for health or human services/benefits.
  4. Six months experience in interpreting laws rules regulations standards policies and procedures.
  5. Six months experience in narrative report writing.
  6. Possession of a valid Drivers License (not suspended revoked or cancelled or disqualified from driving).

The State of Delaware recognizes the value oftelework and flexible working schedules. This position is eligible for Telework and Alternative Work Schedules according to the State of Delawares Alternative Work Arrangement Policy. More information will be shared at the time of interview.

This position is a classification organized under an exclusive bargaining representative (labor organization) that has been elected by employees as their representative for collective bargaining and other applicableterms and conditions of employment in accordance with Title 29 Chapter 59 and Title 19 Chapter 13 and 16. This position is covered by a collective bargaining agreement based on its individual terms.

Preemployment Drug Testing: Upon a conditional offer you must undergo preemployment drug testing as part of the hiring process.
