Logan Healths Woodland Clinic is looking for a parttime Ambulatory Pharmacist to join their team!
About our clinic:
We are situated in a quaint neighborhood of Kalispell MT at 705 6th Avenue East. Although there is not an an onsite dispensing pharmacy at this location we offer by appointment phone and in person patient medication guidance support education and testing. We are located in the heart of the Flathead Valley offering a variety of outdoor recreation and the stunning Glacier National Park.
What youll be doing:
This unique ambulatory care role puts you at the center of chronic disease management education and access to care. In this position you will:
Serve as an extension of the care team supporting providers and patients with medicationrelated questions and care decisions.
Offer personalized education and medication counseling for patients with chronic conditions such as diabetes high cholesterol and hypertension.
Assist with medication prior authorizations and financial assistance applications to improve medication access.
Provide ongoing support for insulin pumps A1C tracking and anticoagulation therapy.
Conduct medication reviews and offer guidance for patients struggling with chronic conditions
Offer telephone support and inperson appointments via provider referrals
Answer patient questions and provide guidance regarding medication options
Who you are:
We are looking for a seasoned experienced pharmacist for this impactful position. Our ideal candidate will hold a current BPS Board Certification. Ambulatory pharmacy experience is a plus but not required! You must be compassionate and have a passion for helping provide exceptional patient care support and education
Qualifications:
PharmD from a College of Pharmacy accredited by American Association of Colleges of Pharmacy or equivalent clinical experience required.
Current Montana State Board of Pharmacy Registered Pharmacist License required.
Board of Pharmacy Specialties (BPS) certification is preferred.
Accurate data entry skills analytical and advanced math skills required.
RQI required and as applicable to assigned area(s):
First Responder required for assignment: Clinic Retail Behavioral Health IS and Administration Pharmacists.
ACLS required for all other assigned area(s).
Demonstrated ability to make independent judgments under stressful situations at times. Must work effectively and efficiently without direct supervision.
Excellent organizational skills detailoriented a selfstarter possess critical thinking skills and be able to set priorities and function as part of a team as well as independently.
Commitment to working in a team environment and maintaining confidentiality as needed.
Excellent verbal and written communication skills including the ability to communicate effectively with various audiences.
Excellent interpersonal skills with the ability to manage sensitive and confidential situations with tact professionalism and diplomacy.
Possess and maintain computer skills to include working knowledge of Microsoft Office Suite and ability to learn other software as needed.
Job Specific Duties:
Evaluates and verifies orders for therapeutic appropriateness proper dosing optimal dosage form drug interactions allergic reactions route and rate of administration and therapeutic redundancy.
Provides patientspecific pharmacotherapy recommendations and drug information to the appropriate stakeholders as applicable to assigned area(s).
Performs daily review of pharmacy reports including pharmacokinetic monitoring as applicable to assigned area(s).
Documents clinical interventions appropriately in electronic medical record.
Implements all drug use policy and formulary decisions of the Pharmacy and Therapeutics committee.
Reviews and updates Collaborative Practice Agreements per department protocol for assigned area(s).
Supports and precepts pharmacy students on practice rotations as applicable to assigned area(s).
Assesses and evaluates the medical and drug literature. Compares and contrasts therapeutic regimens and uses for new or existing drug treatments.
Provides direct and indirect patient care for patients through Collaborative Practice Agreements as applicable to assigned area(s).
Develops and participates in operational and clinical projects as determined by departmental needs and ongoing projects as applicable to assigned area(s).
Performs direct patient care roundingorder verification discharge counseling and education ofother healthcare professionals as applicable to assigned area(s).
Serves as a preceptor for pharmacy students on rotations as applicable to assigned area(s).
Maintains pharmacy knowledge through continuing education and applies new principles to current practice as needed.
Exhibits knowledge of policies and procedures as they relate to job duties and HIPPA.
Coordinates medication ordering preparation dispensing and administration systems for the oncology patient populations as applicable to assigned area(s).

Shift:
Day Shift 8 Hours (United States of America)Schedule: Parttime 20 hours per week. Generally this will be two 8 hour shifts and one 4 hour shift per week.
Logan Health operates 24 hours per day seven days per week. Schedules are set to accommodate the requirements of the position and the needs of the organization and may be adjusted as needed.
