The Associate VP Care Management leads teams of nurses and behavior health professionals responsible for care management. The Associate VP Care Management requires a indepth understanding of how organization capabilities interrelate across segments and/or enterprisewide.
The Associate VP of Care Management directs the strategic development growth and operations of the Illinois Department of Healthcare and Family Services. They have overall accountability to ensure that personcentered integrated care is provided to Illinois Medicare and Medicaid members. They promote these goals by training leading and motivating their teams to be engaged culturally and linguistically competent and committed to building positive relationships with members caregivers families providers CBOS DMAS and other stakeholders.
RESPONSIBILITIES:
- Leads develops and operationalizes care management/coordination within Humanas population health strategy education and quality improvement activities for the clinical team.
- Develops implements measures analyzes and reports on programs and initiatives designed to improve the health and quality of life of our members.
- Ensures best practice standards and the National Committee of Quality Assurance (NCQA) Care Management criteria are used to assist with developing the documentation tools to create impactful member outcomes.
- Responsible for the planning directing and successful management of the Clinical Strategy for Humanas contracts with the Department of Healthcare and Family Services.
- Provides input into functions strategy.
- Leads mentors exemplifies and inspires teams responsible for care management.
- Contributes to the success of associate and member experience in assigned region as well as across the enterprise.
- Oversees the assessment and evaluation of Members needs and requirements to achieve and/or maintain optimal wellness state by guiding Members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of Members.
- Makes decisions typically related to the implementation of new/updated programs or largescale projects for the function and supporting technical/operational procedures and processes and implements strategic plans drives goals and objectives and improves performance.
- Develops and maintains functioning teams to accomplish goals set by the division.
- Encourages professional responsibility and assist with professional development.
- Defines key personnel productivity and quality indicators that meet or exceed industry standards.
- When necessary develops and update process improvement plans for internal and external customers.
- Develops education and training programs for care management team inclusive of case review and case study.
- Collaborates with other Humana leadership to identify the clinical tools and systems to support care management activities.
- Participate in Care Management Collaborative meetings as required by the Illinois Department of Healthcare and Family Services.
- Ensures that the Health Services team operates in compliance with Commonwealth and federal Regulations Commonwealth Contract Requirements Humana Policies and NCQA Standards

Required Qualifications
- Bachelors degree in Psychology Social Work Nursing or related field
- 5 plus years case management experience
- 5 plus years of experience working directly with Medicare and Medicaid programs
- 5 plus years of experience leading and managing people
- Experience in strategic planning and development
- Experience in developing behavioral health care management programs and integrated care management models.
- Demonstrated successful leadership skills in program execution and people management
- Proven leadership skills in both internal and external environments
- Must reside relocate or live within commutable distance to Schaumburg IL or Chicago IL office
Preferred Qualifications
- Strongly preferred Behavioral Health case management
- Strongly preferred experience with Long Term Supports & Services for members who are Aged Blind and Disabled (ABD) and/or those with Intellectual and Development Disability
- Active Registered Nurse (RN) license
- Care Management certification (CCM)
- Master Health Administration or Public Health

Pay Range
The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills knowledge experience education certifications etc.

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
