Job Description
Job Summary
The Wellness Navigator is the point professional providing clinical services for Augusta Healths wellness program. This work includes health screenings and assessments for team members personal goal setting motivational interviewing service referrals and longitudinal coaching support when indicated. The Wellness Navigator also has an educational role creating resources and delivering programs to teach about health and to engage the broad diversity of our team members.
Education
- Bachelors degree in Nursing OR Certification in Health Coaching with 2 years experience as a Health Coach.
Licensure/Certification
- Current and valid Registered nurse license in the Commonwealth of Virginia or from a state that is part of a Compact agreement with Virginia OR Certification in Health Coaching if not a Registered Nurse
Competencies Knowledge Skills and Abilities
Ability to complete health assessments identify priorities in collaboration with team members and effectively coach toward health promotion
Ability to educate others on complex health topics
A strong understanding of the many health dimensions that tie to wellness such as fitness nutrition sleep mental health addiction metabolic health social health determinants and others.
Ability to provide care that is culturally competent and respectful of diverse perspectives and lived experiences
Genuine interest in preventive care and promoting health in our community
Energetic motivated able to work autonomously
Able to maintain professionalism and confidentiality
Possesses excellent verbal and listening skills
Functions independently and exercises appropriate judgement
Able to prioritize assignments and tasks
Demonstrates flexibility in dealing with a variety of situations and personalities
