Job Summary
We are seeking a full time 40-hour Health Navigator to support our Orthopedic practices in a hybrid capacity. Our practices are open Monday through Friday from 8:00am to 5:00pm.
The Health Navigator serves as a contributing member on the Orthopedic Health team. They provide screening and assist with patient disposition plans that present with a health crisis. They help Orthopedic patients connect with resources that allow them to manage orthopedic issue as they arise and improve overall health and potentially aid and recover from surgical procedures hospital. They assist with posting discharge care and connecting patients to community resources and supports for individuals with complex circumstances.
Essential Functions:
-Advocate for clients or patients to resolve crises.
-Oversee Medicaid and Medicare related paperwork and recordkeeping in hospitals.
-Work closely with Medical Case Managers and providers.
-Plan coordinate and facilitate referrals to existing community resources when indicated by assessed need.
-Support of clinical integration needs of other clinical services and collaboration with medical director leadership
-Maintain current patient case records and prepare reports as indicated.
-Assist with client phone calls letters and medical records on behalf of Orthopedic team
-Through service provision and regularly scheduled visits build relationships that models wellness and recovery with assigned clients that result in improved engagement with services.
Education
Bachelors Degree Related Field of Study required
Experience
Knowledge of the communities served and experience working with community resources 2-3 years preferred and Previous Mental health experience 1-2 years required
