The path to healthcare and improving your life is complicated and challenging and even more so for our vulnerable community members who experience homelessness severe mental illness substance use disorder and/or serious medical anIntegrated Care Manager you provide support to these highly vulnerable participants through intensive coordination of health and healthrelated services. You will coordinate care and services among the physical behavioral dental developmental and social services delivery systems making it easier for participants to get the right care at the right time. These services improve access to care for people with complex needs and who are facing difficult life and health circumstances. You will develop a comprehensive care plan with each participant and act as their primary point of contact to navigate the various system of care they need to access to meet their needs. Additionally you will ensure all providers involved in the participants care are communicating with each other and working together. Requires at least one (1) year experience working in the social work field providing direct client services. Position has potential for a flexible schedule some weekend or evening hours could be needed to meet client needs. This position requires compensated use of personal vehicle must have CA drivers license. Bilingual Spanishspeaking candidates strongly desired.
Key Responsibilities:
- Conduct outreach and engagement efforts to connect potential participants to Enhanced Care Management (ECM) benefit.
- Screen potential clients and provide them with an orientation to the ECM benefit.
- Complete intake with ECM clients. Assess and identify needs and draft an individualized care plan with them.
- Works with clients in group and individual settings to discuss break down and resolve issues/barriers interfering with the plan of care.
- Refer and link clients to appropriate medical and mental health care aiding them with scheduling and making it to appointments.
- Aid guests with utilizing emergency services (ambulance emergency room etc.) only as necessary in an emergency. Connect them with ongoing primary care providers and specialized providers as needed.
Qualifications:
- Minimum of one (1) year of experience working in a social workrelated position providing direct client services. Prefer candidates with experience working with people experiencing homelessness.
- Strongly desired Candidates with lived/personal experience with homelessness mental illness substance use disorder and/or other challenges experienced by the people served.
- Must have basic knowledge of Microsoft Suite (Excel PowerPoint Outlook etc.).
- Must be able to provide written verification of experience and education upon hire.
Classifications:
- Care Manager I less than two (2) years of fulltime experience in the social work field providing direct services.
- Care Manager II At least two (2) years of fulltime experience in the social work field providing direct services.
Other Qualifications:
- Must have ability to occasionally or regularly work a flexible schedule including weekends and evenings.
- Strongly desired Candidates that speak and/or are fluent in Spanish or American Sign Language (ASL).
Driving Requirement:
This position requires the use of a personal vehicle for business purposes and oncall rotation.
Other Requirements:
- Driving is a required function of this position. Employees must use a personal vehicle that meet all agency requirements including maintaining liability insurance that meets or exceeds state requirements.
- Employees are responsible for ensuring the timely renewal of their vehicle insurance policies and providing updated documentation to the agency to remain in good standing and compliant with position requirements.
Conditions of Employment:
- May work across all agency programs as needed.
- Occasional flexible work schedule required to meet individual client needs.
- Must possess a valid CA Drivers License current auto insurance and a clean DMV printout (no more than 3 points).
- Must pass a clear background check with DOJ FBI & CCL.
- CPR and First Aid Certified.
- TB Exam and Physical at the time of employment.
Training:
- Preferred Training in harm reduction Housing First traumainformed low barrierservice models.
- RCS provides an indepth onboarding and training orientation which includes CPR/First Aid Crisis Communication/ProACT TraumaInformed Care Motivational Interviewing Collaborative Problem Solving introductions to all RCS departments and other valuable trainings
