Description

In an environment of continuous quality improvement the Community Outreach Specialist is responsible for conducting community health screenings and educational programs that support the health needs of our community. This position also works alongside other services within the department to promote and grow all areas of Community Health and Wellness Lifestyle Medicine and employee wellness programs. This role requires travel to various locations where needed. Exhibits the MHS Standards of Excellence and exercises strict confidentiality at all times.

Job Functions:

  1. Performs health screening tests for the client such as blood glucose blood pressure heart rate oxygen saturation and weight at designated locations. Works to grow outreach screenings to additional locations as determined by need and request.
  2. Support weekly coordination and implementation of FARMacy program activities; work collaboratively with partners and other health professionals to develop and deliver effective educational components conduct related outreach communication and evaluation efforts to the FARMacy program.
  3. Responsible for routine and frequent communication with local farmers farmers markets and food aggregators to ensure consistent and reliable produce delivery and distribution for participants. Uses various communication methods to resolve issues and concerns; acts as liaison between participants director and lead healthcare provider.
  4. Tracks weekly attendance using process behavior charts and adjusts program content curriculum and educational material accordingly to maintain enrollment and participant engagement and enthusiasm.
  5. Assists director and other department staff with class instruction and group education for programs such as FARMacy Lifestyle Medicine Matter of Balance and others as indicated.
  6. Assists in daily operation of fitness facilities as indicated by location. Includes assisting with maintaining fitness center equipment and general facility upkeep.
  7. Interacts with fitness center members to ensure exercise programs are conducted in a safe manner as well as instructing members on proper equipment use. Addresses concerns of fitness center members.
  8. Develop provider engagement strategies and communication for referral into various wellness and Lifestyle Medicine (LM) programs. Ensure efficient process to maximize referrals.
  9. Works with the Director and other department staff to develop promote and grow a large variety of services and programs for the greater community and serves as liaison for community members screening participants 55 PLUS members FARMacy participants LM patients and others seeking information on programs and events.
  10. Coordinate the collection of patient data and provide followup to clients as needed. Ensure all data is maintained and monitored accurately for all programs.
  11. Assists in coordination and implementation of community outreach screenings health fairs speaking events blood drives and educational activities for the community; includes communication with outside organizations as well as internal departments.
  12. Works in conjunction with the Marketing department and other department staff for promotion of events.
  13. Assists Director and other department staff with collection of community benefit (CBISA) occurrence forms and inputs them into Lyon Software CBISA database. Helps to educate MHS staff on CBISA and works closely with Finance department and Director to ensure occurrences comply with IRS standards.
  14. Assumes all other duties and responsibilities as necessary.

Qualifications

Minimum Education/Experience Required:

  • High School Diploma/GED required.
  • Two years of relevant STNA experience preferred.
  • BLS (Basic Life Support) required will obtain during Hospital Orientation required.
  • Must have a valid drivers license and remain insurable by hospitals agent.
  • Required to travel between various locations throughout the health system as assigned.
  • Experience in health wellness or lifestyle coaching or other areas of community health are preferred.

Special Knowledge Skills Training:

  • Verbal clerical numerical and manual dexterity color vision and near visual acuity.
  • Ability to collect vital signs (blood pressure weight oxygen saturation) required; will train upon hiring.
  • Experience with public speaking group instruction class facilitation a plus!

Compensation Details: Education experience and tenure may be considered along with internal equity when job offers are extended.
