Compensation Package:
JOB SUMMARY: The Case Manager provides a range of comprehensive communitybased case management services including housing services and traumainformed case management for individuals who are homeless and/or returning to the community from incarceration. The position serves as a liaison to the District of Columbia Department of Corrections READY Center Anchor Mental Health (Core Service Agency) and Housing to identify and coordinate community resources and make the appropriate referrals. The Case Manager establishes and maintains effective working relationships among area residents and property owners in the community.
ESSENTIAL DUTIES and RESPONSIBILITIES:
Interview and assess the needs for direct services of Returning Citizens (postrelease) and individuals referred by jails or prisons including the DC Jail before discharge (prerelease) primarily in the District of Columbia who have mental health needs and/or a history of addictive behaviors.
Meet with community partners such as the Resources to Empower and Develop You (READY) Center District of Columbia Department of Corrections (DCDOC) Court Services and Offender Supervision Agency for the District of Columbia (CSOSA) Office of Returning Citizens Affairs (ORCA) and other partners to identify areas for collaboration and develop plans for coordinating services.
Ensure participants engage in group sessions that allow them to address and improve personal aspects of their lives such as financial literacy employment readiness identity development relationships family dynamics and conflict resolution.
Conduct housing orientations to clients which include instruction on the Fair Housing Act Rights of Persons with Disabilities and Landlord and Tenant Rights among other topics.
Maintain complete and accurate case records to include entering data in the database within 48 hours of contact/service.
Conduct program orientation sessions for volunteer mentors to introduce the mentor program and program training sessions to improve the skills and knowledge of the mentors.
EDUCATION and EXPERIENCE:
For social work degree social work licensure in the appropriate jurisdiction is required.
SKILLS and COMPETENCIES:
Driving is required. Must have a valid drivers license and clean driving record. Must have own transportation.
