A. GENERAL ACCOUNTABILITY

This position is responsible for the provision of housing access services that assist clients with securing housing within WoodGreen Community Housings portfolio and other external provider partners. Operating within Housing Services this position supports the organizations primary goal of providing access to appropriate affordable housing including homeless people and those living on low to moderate incomes. The philosophy of this work is in keeping with WoodGreens commitment to enable access to affordable housing and participation in the building of healthy sustainable communities.

B.NATURE AND SCOPE

WoodGreens Housing Unit provides low and moderateincome individuals and families including recently homeless and other barriers with housing and supports and /services to enable independent living as a result of stable housing.
This position is primarily responsible for providing supports & services around access to Rent Geared to Income (RGI) affordable and marketrent units within WoodGreen Community Housings portfolio and other partner sources of affordable housing in the rental market. This position will also enable and provide information on access to programs and services internally through other WoodGreen Units such as Financial Empowerment Employment Childcare Counselling Developmental Services Senior Support Services and Community Care Programs.

C.SPECIFIC ACCOUNTABILITIES

Housing Access Responsibilities:
  • Provide support to clients families and community partners in accessing and processing WoodGreen Community Housing applications for independent and support units.
  • Work in collaboration with referral agencies including PATH Access Point Coordinated Access and internal WoodGreen programs to enable and or facilitate housing access to eligible applicants
  • Coordinate intake with applicants and community partners to ensure completeness of applications and supporting documentation required for WCH application.
  • Determining the eligibility of referrals confirming housing to eligible applicants notifying ineligible applicants and providing information around alternative programs
  • Maintain ongoing communication with WCH Housing Unit most notably housing staff at building locations across WCHs portfolio to confirm applications are properly submitted and applicants are being contacted.
  • Maintain ongoing communication with external housing providers/partners most notably with front line staff to confirm applications and intake processes are completed including providing feedback to applicants
  • Take part in team and unit meetings; and participate in regular supervisory sessions with the Quality Assurance Manager.
  • Coach participants so they enhance their skills to access relevant resources and supports in order to ensure longterm tenancy.
  • Refer applicants to translation services and other culturally appropriate services
Access System Management
  • Maintain WoodGreen access List for housing through the PATHS referral process WoodGreen internal processes as well as other Affordable housing options
  • Review referrals files with Team Leader to ensure applicants are income and program eligible for Affordable and Rent to Geared housing
  • Support team leader in evaluating potential applicants on program criteria policies and eligibility requirements and restrictions.
  • Work with Housing Unit in order to identify vacancies and fill all independent WCH vacancies.
  • Interprets and apply program policies relative to access requirements
  • Complete updates and purges to all applicant information to date
  • Undertake the entering of new applications and documenting access applications in the system of record
  • Assist in the creation and development of necessary policies procedures & guidelines as needed.
  • Prepare all data entries related to access and client information are up to date to enable monthly weekly/annual reporting to WoodGreen and funders.
  • Liaise with Affordable housing referral partners to ensure rental rates quoted are current rental rates for that particular address and agreement.
Training & Development activities:
    • Familiarize with policies documents and strategizes with regards to emerging needs and issues in the community for those looking to access independent social housing
    • Participant to community forums/town hall meetings on housing issues.
    • Plan and implement ongoing promotion/awareness building of WCH and its services to the immediate and broader community.
    • Familiarize yourself with WoodGreen partnerships and requirements for housing with supports and Affordable housing partners
    • Participate in internal team building activities and connect with key stakeholders to ensure an integrated customer service focused work culture
    • Ensure the antiracism objectives and community development principles of the organization are achieved within the program.
