Job Description

The Parkinsons Disease Program Coordinator reflects the mission vision and values of NM adheres to the organizations Code of Ethics and Corporate Compliance Program and complies with all relevant policies procedures guidelines and all other regulatory and accreditation standards.

Responsibilities:

  • Coordinates referrals for PT OT and SLP services for Interdisciplinary Evaluations and follow up care based on clinical recommendations from rehab team physician communication and appropriateness of patient referrals.
  • Collaborates closely with patients treatment providers and the patient to develop and implement a treatment plan.
  • Participates in inter and intradepartmental meetings related to the care of patients with Parkinsons disease.
  • Coordinates all aspects of Support Group and team meetings including scheduling speakers developing agenda and speaker list planning yearly schedule food supplies room reservation and setup and collaboration with communitymember Support Group leader.
  • Attends Support Group meetings engages with all participants and families/caregivers and provides schedule and educational handouts for participants.
  • Communicates with representatives from Pastoral Care (leading caregiver support discussion) Health and Fitness (exercise program manager)and other departments within the healthcare system.
  • Communicates evaluation findings and treatment recommendations with Health and Fitness team for safe class participation.
  • Documents per hospital departmental and program standards protocols procedures and adheres to all regulatory agencies mandates.
  • Represents the program with various level of personnel both internally and externally incorporating Patients First focus and goals.
  • Coordinates staffing with clinical leadership for interdisciplinary program evaluations and support group/class schedules.
  • Serves as a liaison on behalf of the program leadership to foster strong working relationships.
  • Collects feedback from clients and physicians and implements changes to improve the experience of all.
  • Develops outcomes measures and tracks/reports on program outcomes.
  • Demonstrates a high level of interpersonal skills initiative integrity and commitment to excellence and team building.
  • Responds to unanticipated complex issues and manages critical and confidential information.
  • Assists the department leadership in maintaining department competencies and staff education.
  • Attends regional Parkinsonsbased educational and networking events (e.g. NM Parkinsons Symposium NPF/Michael J. Fox programming etc..
  • Creates writes edits and posts articles news and program summaries. Overseas development and maintenance of website and NMI content ensuring ease and practicality of use for targeted audience in collaboration with marketing department.
  • Develops internal and external communications including presentations speaking points correspondence and other written materials.
  • Participates in preparing program budget monitors program expenses and funding.
  • Demonstrates a complete understanding of the program funding; creates purchase orders; and processes invoices for payment against purchase order or general expense reimbursement.
  • Pursues grantwriting opportunities for additional program funding.
  • Utilizes advanced Microsoft Office applications to design and develop presentations and reports.
  • Participates in other tasks and other clinical areas in the program as assigned by manager.

Qualifications :

Required:

  • Bachelors of Science in one of the following areas: Physical Therapy Occupational Therapy Speech Language Pathology Nursing or Social Work.
  • Must hold a current Illinois license in one of the above fields.
  • Strong knowledge of Microsoft Office package.
  • Ability to organize and think independently with orientation to deadline and detail.
  • Strong interpersonal skills.

Preferred:

  • Masters degree and program development experience.
  • Experience in the clinical care of patients with Parkinsons disease
