Job Description
Join our dynamic Home Health team as a dedicated Physical Therapist where your expertise will be pivotal in transforming the lives of patients recovering from illness injury or surgery. This fulltime role focuses on delivering exceptional patientcentered care directly in the comfort of their homes.
Key Responsibilities:
- Conduct comprehensive physical assessments and review medical histories to evaluate patients conditions.
- Create and implement personalized treatment plans tailored to each patients unique needs and objectives.
- Administer therapeutic exercises and training programs to enhance patient recovery.
- Provide education to patients and their families on injury prevention wellness and the recovery process.
- Monitor document and report on patients progress towards their rehabilitation goals.
- Collaborate with multidisciplinary healthcare teams to ensure comprehensive patient care.
- Maintain accurate and compliant patient records and documentation.
Qualifications:
- Masters degree in Physical Therapy.
- Minimum of 1 year of experience in home health as a physical therapist.
- Current physical therapy license within the state of practice adhering to professional standards and practices.
- Valid CPR certification maintained throughout the clinical tenure.
- Licensed driver with the ability to travel to various business locations.
