Summary of Job Duties:
The Registered Nurse will promote wellness; improve outcomes and selfcare within a defined population of patients. The RN will provide patients who have chronic conditions the support education and assistance in the prevention and/or maintenance of their disease and/or health and wellness state. Will work with the individual patient their families and providers of care to develop and implement individual health plans and provide support to the patient in selfmanagement. Coordinate care and/or followup for patients who receive care in outpatient facilities. Coordination of prescription refill requests and patient education coordination in collaboration with physician staff. Commitment to patient outcomes results in high levels of performance despite workload changes
Qualifications:
Minimum:
Graduate of an accredited Nursing Program with Associates Degree Nursing Diploma Bachelors in Nursing or Masters in Nursing.
Valid RN License.
One 1 year Registered Nursing experience in ICU or ER.
Current BLS and ACLS Certification
Excellent communication skills.
Basic computer skills.
Strong work ethics and accountability.
Employees in this role must be fitted for capable of donning and performing routine tasks in personal protective equipment to include N95 masks Powered Air Purifying Respirators (PAPR) or Controlled Air Purifying Respirators (CAPR).

Responsibilities:
Performs triage assessment planning implementation and evaluation of nursing care and patient education (in person or by phone).
Delivers nursing care per UAMS ANA AR State Board of Nursing and specialty organization standards.
Administers medications in accordance with clinic standards and the Nurse Practice Act.
Assists in developing the patients Plan of Care (POC) using assessment data.
Evaluates patients progress toward achieving desired outcomes.
Coordinates care and followup for patients in inpatient and outpatient settings.
Implements care plans using an interdisciplinary Patient and Family Centered Care approach.
Advocates for patients and families to uphold their rights and ensure safe competent care.
May perform other duties as assigned.
