To request a new myki card or renewal of an existing pass please click on the apply button and fill in the online form by 11:55pm Wednesday 21st May 2025. Please note due to strict cut off times by PTV any forms received outside of PageUp and/or after this date will not be processed.
Discounted yearly costs current as at 1 January 2025:

Important note to Commuter Club Members who have had their card for more than two years:
Due to Commuter Club pass expiry dates staff who have held their card for more than two years but less than three should apply for a new card rather than having a pass loaded to their existing card. This will ensure your card does not expire throughout the duration of their 365 day pass. Staff can check their card expiry date on a myki machine myki check or via online account: any queries you may have please refer to the myki Commuter Club Frequently Asked (FAQs). Updates to the FAQs document includes information about how you can now obtain an onthespot replacement for damaged/defective cards and information about how refund applications are calculated.
