Annual Salary: $83088.00 $111816.00

Schedule: 8:00AM4:30PM MonFri Sat/Sun off

The purpose of the PSW3 position is to work collaboratively with internal and external stakeholders to provide medical behavioral health and substance use disorder transition planning services. This position directly supports the Departments mission to improve public safety by assessing transition needs of incarcerated individuals preparing for release coordinating and advocating for needed services providing resources and developing Continuity of Care Plans that assist patients with significant physical and/or mental disorders in their transition back to the community.
PSW services are grounded in DOC agency core values of cultivating an environment of integrity and trust and respectful and inclusive interactions. Continuity of Care helps patients succeed which reduces recidivism. This approach contributes to in prison and community safety and supports the DOC commitment to Equity Diversity Inclusion and Respect.
Continuity of Care strives to promote/maintain the medical/mental health stability achieved while incarcerated and encourages community support to the released patients with the goals of successful community reentry and reduction of recidivism. This position in collaboration with the facility Health Services reentry team focuses on the Departments Strategic Goal of Continuity of Care Planning for Health Services patients. Project position paid for from Residential Substance Abuse Treatment (RSAT) grant and High Intensity Outpatient program with focus on providing comprehensive healthcare transition planning for individuals who participated in RSAT program at Washington Corrections Center for Women and Mission Creek Corrections Center for Women.

Duties

Conduct release planning for incarcerated individuals who have healthcare follow up needs.

  • Identify transition needs for incarcerated individuals and assist with identification of community resources as needed.
  • Complete Reentry Needs Assessment healthcare related reentry facilitation and Health Services Reentry plan.
  • Conduct reentry groups and forums as assigned/appropriate based on setting.
  • Set followup medical and behavioral health services in the community
  • Contacting and coordinating services with other agencies including Developmental Disabilities Administration (DDA) and DSHS Home and Community Services (HCS).
  • Coordinate care with the Managed Care Organization (MCO).
  • Attend Multidisciplinary Facility Risk Management Team (MDT/FRMT) meetings.
  • Case conferences with the primary therapist psychiatric provider primary care provider Reentry/Discharge Nurse and/or pharmacy as needed in preparation for release.
  • Provide education about Narcan administration
  • Facilitate Durable Medical Equipment (DME) necessary for immediate support at release.
  • Document all contacts with patients per policy.
  • Respond to kites and other correspondence.
  • Input OMNI encounters.
Facilitate Affordable Care Act (ACA) Transitions Outreach ABD Program. (TOAP) /Expedited Medical Eligibility (EME) and/ or Reentry Community Services Program (RCSP) services prior to release.
  • Offer assistance in completion of all documentation needed for the ACA application process.
  • EME application for identified incarcerated individuals (both residential and outpatient) request Behavioral Health Discharge Summary from Psychologist 4 or assigned primary therapist and follow TOAP procedures.
  • Participate in multidisciplinary team meetings and assist as needed for RCS cases.
  • Understand the Supplemental Security Income (SSI) process and communicate resource information.
  • Coordinate with DSHS Home and Community Services.
  • Participate in networking and information sharing with various community stakeholders.
  • Support medical and mental health staff by coordinating medical surrogacy/legal guardianship when needed.
Other transition and continuity of care duties as assigned.

Qualifications

Required Qualifications:

  1. Must be able to obtain and maintain status as an Agency Affiliated Counselor** through the Washington State Department of Health or possess a current valid WA Social Work license. In addition SW licensure must be maintained in good standing at all times to perform SW duties.
Desired Qualifications:
  1. Licensed Social Worker (independent or associate) in good standing at all times OR Master of Social Work from a program whose accreditation is recognized by the US Department of Education or the Council for Higher Education Accreditation (CHEA) and the ability to obtain/maintain status as a LAAC OR Bachelor of Social Work with a masters in a closely related field and the ability to obtain/maintain status as a LAAC.
  2. 3 to 4 years of postgraduate experience providing professional case management services discharge/release planning and/or professional counseling services (i.e. individual/group crisis intervention psychoeducational) to adults with severe and persistent mental illness criminal/violent behavior and/or chemical dependency issues.
  3. Current valid Washington State Drivers License.

Insurance Benefits

Retirement and Deferred Compensation
State Employees are members of the Washington Public Employees Retirement System (PERS). New employees have the option of two employer contributed retirement programs. For additional information check out the Department of Retirement Systems web site.
Employees also have the ability to participate in the Deferred Compensation Program (DCP). This is a supplemental retirement savings program (similar to an IRA) that allows you control over the amount of pretax salary dollars you defer as well as the flexibility to choose between multiple investment options.
Social Security
All state employees are covered by the federal Social Security. and Medicare systems. The state and the employee pay an equal amount into the system.
Public Service Loan Forgiveness

Holidays
Fulltime and parttime employees are entitled to paid holidays and one paid personal holiday per calendar year.
Note: Employees who are members of certain Unions may be entitled to additional personal leave day(s) please refer to position specific Collective Bargaining Agreements for more information.
Fulltime employees who work full monthly schedules qualify for holiday compensation if they are employed before the holiday and are in pay status for at least 80 Non overtime hours during the month of the holiday; or for the entire work shift preceding the holiday.
Parttime employees who are in pay status during the month of the holiday qualify for the holiday on a prorata basis. Compensation for holidays (including personal holiday) will be proportionate to the number of hours in pay status in the month to that required for fulltime employment excluding all holiday hours. Pay status includes hours worked and time on paid leave.
Sick Leave
Fulltime employees earn eight hours of sick leave per month. Overtime eligible employees who are in pay status for less than 80 hours per month earn a monthly proportionate to the number of hours in pay status in the month to that required for fulltime employment. Overtime exempt employees who are in pay status for less than 80 hours per month do not earn a monthly accrual of sick leave.
Sick leave accruals for parttime employees will be proportionate to the number of hours in pay status in the month to that required for fulltime employment. Pay status includes hours worked time on paid leave and paid holiday.
Vacation (Annual Leave)
Fulltime employees accrue vacation leave at the rates specified in WACor the applicable collective bargaining agreement (CBA). Fulltime employees who are in pay status for less than 80 Non overtime hours in a month do not earn a monthly accrual of vacation leave.
Parttime employees accrue vacation leave hours in accordance with WACor the applicable collective bargaining agreement (CBA) on a pro rata basis. Vacation leave accrual will be proportionate to the number of hours in pay status in the month to that required for fulltime employment.
Pay status includes hours worked time on paid leave and paid holiday.
As provided in WACan employer may authorize a lumpsum accrual of vacation leave or accelerate the vacation leave accrual rate to support the recruitment and/or retention of a candidate or employee for a Washington Management Service position. Vacation leave accrual rates may only be accelerated using the rates established WAC.
Note: Most agencies follow the civil service rules covering leave and holidays for exempt employees even though there is no requirement for them to do so. However agencies are required to adhere to the applicable RCWs pertaining holidays and leave.
Military Leave
Washington State supports members of the armed forces with 21 days paid military leave per year.
Bereavement Leave
Most employees whose family member or household member dies are entitled to three 3 days of paid bereavement leave. In addition the employer may approve other available leave types for the purpose of bereavement leave.
