Job Description
We are seeking a highly organized and compassionate Patient Care Coordinator to join our mental health practice. This vital role serves as the first point of contact for patients and plays a key part in ensuring smooth daily operations and continuity of care across therapy and medication management services.
Key Responsibilities:
- Greet and assist patients in a professional welcoming manner both in person and over the phone
- Schedule appointments for therapists and physician assistants
- Verify insurance eligibility and benefits for new and returning patients
- Coordinate with the billing department to collect required patient demographic and financial information
- Communicate with physician assistants (PAs) pharmacies and providers to relay messages and call in prescriptions as directed
- Maintain accurate and uptodate patient records in the practice management system
- Serve as a liaison between patients providers and external partners to facilitate seamless care
- Manage appointment reminders followup calls and rescheduling needs
- Support general office duties including answering phones checking voicemails and organizing documentation
Qualifications:
- Prior experience in a medical or mental health office preferred
- Strong communication and organizational skills
- Knowledge of insurance verification and scheduling systems
- Comfortable handling sensitive patient information with discretion and professionalism
- Familiarity with TherapyNotes EMR and coordination of care across multiple disciplines
