Job Description

CARE COORDINATOR SPECIALIST

Adagio Health Inc is looking to hire one 1 fulltime Care Coordinator Specialist for its Erie office.

We are looking for compassionate and driven individuals to join our team as a Care Coordinator Specialist. This role is integral to our mission of providing comprehensive support and care to a diverse range of clients including those with specific needs such as HIV veterans and others requiring specialized services.

RESPONSIBILITIES

  • Client Assessment and Support: Conduct thorough assessments of client needs focusing on behavioral health food insecurity transportation housing and other essential community services.
  • Knowledge Development: Gain indepth knowledge of available community support services and programs including the referral process for each.
  • VeteranSpecific Expertise: Become adept in military benefits documentation and health insurance navigation to support our veteran community effectively.
  • Referral Management: Provide wellcoordinated referrals both internally and externally ensuring a seamless transition for clients.
  • Barrier Identification and Resolution: Assist clients in identifying and overcoming barriers to service access utilizing local resources and supports to optimize care.
  • Educational Support: Offer education and resources to clients as needed enhancing their understanding and engagement in their own care.
  • Documentation and Data Management: Meticulously document all client interactions and data participating in data analysis and reporting.
  • Quality Improvement Participation: Engage in initiatives aimed at enhancing the quality of services provided.
  • Collaboration and Outreach: Work closely with clinical and program staff maintaining open communication lines. Conduct outreach to build and maintain relationships with community partners.
  • Veteran Mental Health First Aid (MHFA): Obtain and provide Veteran MHFA to partners and veteran audiences.
  • FollowUp: Ensure continuous care by providing consistent followups with patients clients and partners.
  • Teamwork: Collaborate with various community service providers including clinical nutrition and ancillary support services.
  • Technology Utilization: Employ virtual/telehealth and texting program equipment for remote client interactions.
  • Additional Responsibilities: Be adaptable to perform other duties as assigned.

QUALIFICATIONS FOR A CARE COORDINATOR SPECIALIST

  • Bachelors degree in social work sociology human services psychology education or related field.
  • Experience in case management/care planning customer service and enrollment process for medical insurance products preferred.
  • Must be able to comfortably work with Microsoft office products.
  • Must have excellent communication skills. Some regional travel required.
