Job Description
Physical Therapist Home Health $105K$115K Taylor MI
Were seeking a dedicated and compassionate Physical Therapist to join our home health team. In this rewarding role youll have the opportunity to make a meaningful difference in patients lives by providing personalized care in their homes helping them regain independence and improve their quality of life.
Core Responsibilities:
- Perform thorough physical assessments and evaluate patients conditions in their home environment
- Develop and implement individualized treatment plans aligned with patient goals
- Deliver evidencebased therapeutic exercises and rehabilitation programs
- Guide patients and families through injury prevention and wellness strategies
- Track and document patient progress using electronic health records
- Coordinate care plans with nurses occupational therapists and other healthcare professionals
- Maintain compliance with healthcare regulations and professional standards
Required Qualifications:
- Masters degree in Physical Therapy from an accredited program
- Current state Physical Therapy license in good standing
- Minimum 1 year of home health experience
- Valid CPR certification
- Clean driving record and reliable transportation
- Strong interpersonal and communication skills
- Proficiency in electronic documentation systems
