Licensed Medical Social Worker PRN or parttime
Care IV Home Health is a locally owned and operated home health agency in Hot Springs AR seeking a parttime or PRN Physical Therapist to join our esteemed team. Requirements:
- Medical Social Worker actively licensed or registered in the state of Arkansas.
- A minimum of three professional references on the application or upon initial contact.
Duties may include but are not necessarily limited to:
- Provide rehab and supportive casework geared towards restoring patients to their optimal level of social emotional & health wellness
- Help patients utilize appropriate family & community resources
- Refer patients to appropriate health & welfare agencies
- Consult & coordinate services with physicians and other members of the patients care team
- Prepare clinical progress notes & incorporate into clinical record
- Participate in discharge planning & inservice programs
